適應症

Adjunct to diet for the treatment of patients with elevated total-C, LDL-C, apo B & TG & to increase HDL-C in patients with primary hypercholesterolemia (heterozygous familial & non-familial hypercholesterolemia), combined (mixed) hyperlipidemia (Fredrickson types IIa & IIb), elevated serum triglyceride levels (Fredrickson type IV), & for patients with dysbetalipoproteinemia (Fredrickson type III) who do not respond adequately to diet. Reduction of total-C & LDL-C in patients with homozygous familial hypercholesterolemia when response to diet & other non-pharmacological measures is inadequate. Secondary prevention of the combined risk of death, non-fatal MI, cardiac arrest & re-hospitalization for angina pectoris in patients with CV disease &/or dyslipidemia. Adjunct to diet to reduce total-C, LDL-C & apo B levels in boys & postmenarchal girls 10-17 years with heterozygous familial hypercholesterolemia if after an adequate trial of diet therapy the following findings are present: LDL-C remains ≥ 190 mg/dL or LDL-C remains ≥ 160 mg/dL & there is a positive family history of premature CV disease or 2 or more other CVD risk factors are present.

用量

Dose range: 10-80 mg once daily. Primary hypercholesterolemia & combined (mixed) hyperlipidemia Usually 10 mg once daily. Homozygous familial hypercholesterolemia Usually 80 mg once daily. Heterozygous familial hypercholesterolemia in children 10-17 years Initially 10 mg/day. Max dose: 20 mg/day.

May be taken with or without food

Category X: Studies in animals or human beings have demonstrated foetal abnormalities or there is evidence of foetal risk based on human experience or both, and the risk of the use of the drug in pregnant women clearly outweighs any possible benefit. The drug is contraindicated in women who are or may become pregnant.

禁忌

Active liver disease or persistent elevations of serum transaminases > 3 times the upper limit of normal. Pregnancy & lactation.

注意事項

History of liver disease or heavy alcohol ingestion. Perform liver function tests before treatment & periodically thereafter. Should an increase in ALT or AST of > 3 times the upper limit of normal persist, reduce dose or withdraw drug. Patients should be advised to report promptly any unexplained muscle pain, muscle tenderness or weakness, especially if accompanied by malaise or fever. Discontinue if markedly elevated CPK levels occur or myopathy is diagnosed or suspected.

不良反應

Constipation, flatulence, dyspepsia, abdominal pain, headache, nausea, myalgia, asthenia, diarrhea, insomnia.

交互作用

Risk of myopathy increased with concurrent administration of cyclosporin, fibric acid derivatives, erythromycin, niacin or azole antifungals. Decreased atorvastatin plasma concentration with oral antacid suspensions containing Mg & Al hydroxides & colestipol. Increases steady-state plasma digoxin conc. Increased atorvastatin plasma concentration with erythromycin, clarithromycin & protease inhibitors. Increases AUC values for norethindrone & ethinylestradiol.
