成份 Tacrolimus
適應症 Primary treatment for adult liver & kidney transplantation or secondary treatment for adult liver & kidney transplantation which has no effect after treating with cyclosporin.
用量 Prophylaxis of kidney transplant rejection Initially 0.2-0.3 mg/kg/day once daily in the morning within 24 hr after surgery. Prophylaxis of liver transplant rejection Initially 0.1-0.2 mg/kg/day once daily in the morning approximately 12-18 hr after surgery.
用法 Should be taken on an empty stomach (Take on an empty stomach at least 1 hr before or 2-3 hr after meals.).

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus. 
禁忌 Hypersensitivity to macrolides. Lactation.
注意事項 Monitor significant changes in BP, ECG, neurological & visual status, fasting blood glucose levels, electrolytes (particularly potassium), liver & renal function tests, hematology parameters, coagulation values & plasma protein determinations, EBV-PCR during initial post-transplant period. Concomitant administrantion with St. John's Wort or other herbal prep, cyclosporin. Episodes of diarrhea. Monitor high-risk patients receiving substantial immunosuppression using echocardiography or ECG pre- & post-transplant (eg initially at 3 months & then 9-12 months). Diagnosed or suspected Congenital Long QT Syndrome. Perform EBV-VCA serology before use. Limit exposure to sunlight & UV light. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May impair ability to drive or operate machinery. Pregnancy.
不良反應 Tremor, headache, diarrhea, nausea, renal impairment, hyperglycemic conditions, DM, hyperkalemia, HTN, insomnia. Ischemic coronary artery disorders, tachycardia; anemia, leukopenia, thrombocytopenia, leukocytosis, abnormal red blood cell analyses; CNS disorders; blurred vision, photophobia, eye disorders; tinnitus; respiratory, thoracic & mediastinal disorders; GI disturbances; renal & urinary disorders; pruritus, rash, alopecia, acne, increased sweating; arthralgia, muscle cramps, limb & back pains; metabolism disorders; primary graft dysfunction; vascular disorders; asthenic conditions, febrile disorders, edema, pain & discomfort, increased blood alkaline phosphatase, increased weight, disturbed body temperature perception; hepatic enzymes & function abnormalities, cholestasis & jaundice, hepatocellular damage & hepatitis, cholangitis; psychiatric disorders.

交互作用 CYP 3A4 inducers & inhibitors; antifungal agents eg ketoconazole, itraconazole, voriconazole; erythromycin; HIV protease inhibitors eg. ritonavir; bromocriptine, cortisone, dapsone, ergotamine, gestodene, lidocaine, mephenytoin, miconazole, midazolam, nilvadipine, norethindrone, quinidine, tamoxifen, (triacetyl)oleandromycin, grapefruit juice, lansoprazole, cyclosporin; rifampicin, phenytoin, St. John's Wort; phenobarbital, corticosteroids, carbamazepine, metamizole, INH; prokinetic agents eg metoclopromide, cisapride; nephrotoxic or neurotoxic drugs eg aminoglycosides, gyrase inhibitors, vancomycin, cotrimoxazole, NSAIDs, ganciclovir or acyclovir, amphotericin B, ibuprofen; K-sparing diuretics; live attenuated vaccines; oral anticoagulants & antidiabetics.
