適應症

In combination with docetaxel for the treatment of locally advanced or metastatic breast cancer after failure of cytotoxic chemotherapy that included an anthracycline. As monotherapy for the treatment of locally advanced or metastatic breast cancer after failure of a taxane & an anthracycline-containing chemotherapy regimen or for whom further anthracycline therapy is contraindicated. Adjuvant treatment of patients following surgery of stage III (Dukes' stage C) colon cancer & as 1st-line treatment of patients with metastatic colorectal carcinoma. In combination with platinum as 1st-line treatment of patients with advanced gastric cancer.

用量

Monotherapy Colon, colorectal & breast cancer 1,250 mg/m² bid for 14 days followed by a 7-day rest period. Combination therapy Colorectal & gastric cancer Reduce starting dose to 800-1000 mg/m² bid for 14 days followed by a 7-day rest period or to 625 mg/m² bid when administered continuously. Breast cancer 1,250 mg/m² bid for 14 days followed by a 7-day rest period, combined with docetaxel 75 mg/m² as a 1-hr IV infusion every 3 weeks. Premed with oral corticosteriod should be started prior to docetaxel administration.

過量

Should be taken with food (Take within ½ hr after meals.).

Category D: There is positive evidence of human foetal risk, but the benefits from use in pregnant women may be acceptable despite the risk (e.g., if the drug is needed in a life-threatening situation or for a serious disease for which safer drugs cannot be used or are ineffective).

禁忌

History of severe & unexpected reactions to fluoropyrimidine therapy or hypersensitivity to fluorouracil. Dihydropyrimidine dehydrogenase (DPD) deficiency. Pregnancy & lactation. Severe leucopenia, neutropenia, or thrombocytopenia. Severe hepatic/renal impairment. Treatment with sorivudine or its chemically related analogues, such as brivudine. If contraindications exist to any of the agents in the combination regimen, that agent should not be used.

注意事項

Monitor for severe diarrhea. Dehydration should be prevented or corrected at the onset for patients with anorexia, asthenia, nausea, vomiting or diarrhea. Hand-foot syndrome. Avoid administration of pyridoxine for for symptomatic or secondary prophylactic treatment of hand-foot syndrome if use in combination with cisplatin. History of CHD, significant cardiac disease, arrhythmias & angina pectoris; monitor for cardiotoxicity. Pre-existing hypo- or hypercalcemia. Central or peripheral nervous system disease, eg. brain metastasis or neuropathy. DM or electrolyte disturbances. Monitor prothrombin when co-administer with oral coumarin-derivative anticoagulant therapy. Mild to moderate liver dysfunction, regardless of the presence or absence of liver metastasis. Therapy should be interrupted if treatment-related elevations in bilirubin of > 3.0 x ULN or treatment-related elevations in hepatic aminotransferases (ALT, AST) of > 2.5 x ULN occur. Moderate renal impairment. Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. May impair ability to drive or operate machinery.

不良反應

GI disturbances (especially diarrhea, nausea, vomiting, abdominal pain, stomatitis), hand-foot syndrome (palmar-plantar erythrodysesthesia), fatigue, asthenia, anorexia.

交互作用

Coumarin anticoagulants, phenytoin, sorivudine & its analogues eg brivudine. Allopurinol. Potential interactions with isozymes 1A2, 2C9 and 3A4
