適應症 |
In combination with cisplatin for the treatment of malignant pleural mesothelioma. Monotherapy for the 2nd-line treatment of locally advanced or metastatic non-small cell lung cancer (NSCLC) (other than predominantly squamous cell histology). In combination with cisplatin therapy for the 1st-line treatment of locally advanced or metastatic nonsquamous NSCLC (other than predominantly squamous cell histology). Monotherapy for the maintenance treatment of locally advanced or metastatic NSCLC other than predominantly squamous cell histology in patients whose disease has not preogressed immediately following platinum-based chemotherapy. First-line treatment should be a platinum doublet with gemcitabine, paclitaxel or docetaxel. |

用量 |
500 mg/m2 by IV infusion over 10 mins on Day 1 of each 21-day cycle. In malignant mesothelioma, cisplatin (75 mg/m2) is given 30 mins after the end of pemetrexed infusion. |

注意事項 |
Moderate to severe renal or hepatic impairment. Pregnancy & lactation. Monitor FBC. Folic acid & vit B12 supplementation required to reduce treatment-related hematologic & GI toxicity. Pre-treatment with a corticosteroid (e.g. oral dexamethasone) reduces the incidence & severity of cutaneous reactions. |

不良反應 |
Bone marrow depression with neutropenia, leukopenia, anemia & thrombocytopenia; fatigue; fever; GI toxicity with nausea & vomiting, constipation, anorexia, stomatitis/pharyngitis, diarrhea; dyspnea; chest pain; infection; rash/desquamation.

交互作用 |
Concomitant administration of nephrotoxic drugs or substances which are tubularly excreted (e.g. probenecid) may delay renal clearance of pemetrexed. NSAIDs may decrease pemetrexed elimination especially in patients with mild to moderate renal impairment. Avoid using NSAIDs 2 days before until 2 days after pemetrexed use, & NSAIDs that have longer half-lives, should be avoided from 5 days before until 2 days after pemetrexed.
