近日，日本卫生劳动福利部（MHLW）已批准三合一复方新药Biktarvy（bictegravir 50mg/emtricitabine 200 mg/tenofovir alafenamide 25 mg，BIC/FTC/TAF），该药是一种每日口服一次的单一片剂方案（STR），用于治疗HIV-1感染。Biktarvy是吉利德将要直接通过其当地子公司Gilead Sciences K.K.在日本推出并商业化的首款HIV产品。

药用药理学
1. 作用机制
Victegravir: Victorigravir 是一种集成酶抑制剂,与整数的活性位点结合,抑制艾滋病毒复制周期所需的逆转录病毒DNA的嵌入过程中DNA链的转移。
Etrisitabine 50-52:etricitabine 是一种基于核酸的逆转录酶抑制剂 (NRTI),一种由细胞内酶引起的磷酸化二聚酰亚比5' - 三磷酸盐。Etricitabine 5'-三磷酸盐通过HIV逆转录酶进入病毒DNA来抑制HIV复制,以阻止DNA链的伸长。
特诺霍韦阿拉菲酰胺:特诺霍韦阿拉菲酰胺是NRTI,特诺韦的一种原药。Tenohovir araphen酰胺通过PBMC和巨噬细胞中的儿皮素A将泰诺韦输送到细胞中进行水解。 此后,在细胞中进行磷酸化,变成二磷酸二磷酸二磷酸二磷酸二磷酸二磷酸二磷酸二磷酸酯,通过HIV逆转录酶吸收病毒DNA来抑制HIV复制,以阻止DNA链的伸长。
2. 抗病毒活性
Victegravir,通过评估细胞培养系统中的乙酰氨基甲酰胺和十诺韦阿非酰胺,观察到协同抗病毒活性。

3. 耐药性
(1) 体外测试
Victegravir:在HIV-1菌株对Victorievil的易感性下降,M50I,R263K和S153F突变被观察到。
Emetlycitabine 65: 观察到对易感性降低与Hiv-1逆转录酶的M184V/I突变有关。
泰诺霍韦阿林酰胺:在HIV-1分离菌株易感性对泰诺韦阿非酰胺降低,K65R突变表达,K70E突变也被瞬时观察到。
临床试验
HIV-1传染病患者没有抗HIV药物治疗经验:在1489年和1490项试验的综合分析中,当被确定为病毒性失败时,HIV-1 RNA在服用48周或200份早期中止后流产 在8例mL或以上病例的基因型和表型分析中,观察到新的抗突变的表达。

4. 交叉公差

特诺霍韦阿林酰胺:HIV-1菌株与K65R和K70E突变,阿巴卡韦,吉达诺辛,拉米夫丁,表现出对埃特里他汀和特诺霍韦的敏感性下降,以保持对齐多夫丁的易感性。Q151M复杂突变含有T69S双插入突变或K65R,HIV-1表现出对NRTI的多药耐药性,对二诺霍韦阿拉菲内酰胺的敏感性下降。

About the action and effect of this medicine
Inhibits the activity of HIV-1 reverse transcriptase and suppresses the growth of infectious viruses.

However, if it is almost time for the next dose, skip the missed dose and skip one dose. After that, please take one dose without increasing the amount of medicine from the indicated time. Never drink two doses at once.

Do not stop taking at your own discretion without the instructions of your doctor.

This drug is not a root treatment for HIV infection. Even if you take this medicine, your disease may progress. Please report any changes in your physical condition to your doctor.
