Intelence（Etravirine）是一种用于治疗人类免疫缺陷病毒（HIV）感染的药物。属于一组被称为非核苷逆转录酶抑制剂（NNRTI）的抗HIV药物。用于治疗感染艾滋病毒的成年人和至少2岁的儿童，以及以前使用过其他抗艾滋病毒药物的成年人。

药用药理学
1. 作用机制
埃特拉韦林是NNRTI治疗人体免疫缺陷病毒1型(HIV-1)。埃特拉韦林直接与逆转录酶 (RT)结合, 通过失活dna聚合酶的催化位点来抑制rna依赖性和dna依赖性DNA聚合酶作用。 埃特拉维林与RT结合至少两种不同的构象。埃特拉韦林易于扭曲, 可以采取多种构象, 也可以在耦合口袋中进行重大的重新定位和重新定位, 因为结构紧凑。埃特拉韦林不抑制人 DNA 聚合酶α、β和γ。
2. 抗病毒作用
埃特拉韦林、T细胞系、人外周血单个核细胞对实验室菌株和急性感染人单核细胞巨噬细胞的临床分离株均有活性, EC50值为0.9 ~ 2.4 Ngml (0.4 ~ 2.4 ngml)。Etravirin 是一组广泛的HIV-1 m分离株(A、B、C、D、E、f、G), 表明o组原代分离株的体外抗病毒活性为0.7 ~ 21.7 nM。这些EC50 值足够低于 15 ~ >100 [MU] m 是一种细胞毒性浓度。在人类血清中, Etravirin EC50值对 HIV-1 的影响上升了5.8倍。
Etravirin是PI(放大器Lena Building, Ataza Nabil, Darunavir, Indinavir, Darunavir, Nerfinavir, Riliavir, Tipranavir 和 Sakisavir), N (t) RTI (Zalcitabine, Didanicin, Samilbujin, Avavir 和 Tenofovir)、NNRTI (Efavenz、Delarvir jin 和 Nevirapine) 和融合抑制剂 (enfuvirtit) 和添加剂作用, NRTI (Didivudine、Lamavazine 和 Emtricicabine) 表现出协同作用。 对抗逆转录病毒药物的抗逆转录病毒药物之间没有拮抗作用。
3. 耐药
在引入单一nnrti耐药相关氨基酸突变(包括发生频率最高的K103N和Y181C突变)的65种 hivrin 菌株中, Etravirin 对56株的反病毒活性表现出抗病毒活性。承认最明显下降的突变是Y181I[EC50值的比率表示表型抗折变化(FC)=13]和Y181I(FC=17)。对于24种具有多株N(t)rti和/或PI耐药相关突变的HIV-1菌株, Etravirin的抗病毒作用与野生型HIV-1菌株的活性相当。
除了不同来源和亚型的野生型HIV-1外, 还从NNRTI耐药HIV-1菌株中分离出抗性埃特拉韦林菌株。对Etavirin敏感性的降低, 通常需要在逆转录酶中出现多个突变, 其中L100I、E138K、E138K、V179I、Y181C和M230I是高频观察到的。
在 DUET-工DET-2 测试中, 在含有药物的组合治疗中, 经常出现的病毒治疗效果不足的患者是 V179F、V179F、Y181C、Y181C、 这些经常遇到与其他多个NNRTI 相关突变。 在检测中服用药物的患者中, 最常见的突变是L100I、E138G、V179F、V179F、Y181C、h221y。不到10% 的患者从病毒效应分离不足, 其他NNRTI抵抗相关突变出现的 Etravirin 管理, K101E, K101E, v106iem2, K101E, Y188L, Y188L, g190sc/ 是 C 和 R356K。用Etravirin给药表达的NNRTI突变参与了Etravirin敏感性的降低, Etravirin FC 值是参考值的 40倍, 从基线增加到6倍。
4. 交叉阻力
在引入NNRTI抗性氨基酸突变的65种HIV-1菌株中, 在3个菌株中, 埃特拉韦林和埃法维恩茨之间显示出交叉耐药, 剩余菌株的突变、对埃特拉韦林和埃菲伦的易感性分别降低。
在6171株对Delaville jin、Efavirenz和Nevirapine 具有耐药性的6171株临床分离株中, 83%的ec50值低于10nM。 DUET-ENDUET-2测试灵敏度降低到35%的细胞系分离期间的基线 (FC > 3)被发现, 类似于61%的分离物, 71% 和 79% de la ville jin, Efavenz, 观察到对Nevirapine的敏感性。关于与包括Etravirin 在内的综合治疗中治疗效果不足的患者分离的细胞系, 预计Delarvir jin、Efavirenz或nevirapine会发生交叉耐药。

HIV-1感染症
用法与用量
成人口服一次200毫克, 每天两次作为埃特拉韦林。在服用时, 一定要将其与其他抗艾滋病药物一起使用。

Effects of this medicine
This medicine is a reverse transcriptase inhibitor that inhibits the action of DNA polymerase on human immunodeficiency virus type-1 (HIV-1) to slow disease progression.
It is usually used for treatment of HIV-1 infection in combination with other anti-HIV agents.
Before using this medicine, be sure to tell your doctor and pharmacist
•If you have previously experienced any allergic reactions (itch, rash, etc.) to any medicines.
•If you are pregnant or breastfeeding.
•If you are taking any other medicinal products. (Some medicines may interact to enhance or diminish medicinal effects. Beware of over-the-counter medicines and dietary supplements as well as other prescription medicines.)
Dosing schedule (How to take this medicine)
•Your dosing schedule prescribed by your doctor is <<to be written by a healthcare professional>>
•In general, for adults, take 2 tablets (200 mg of the active ingredient) at a time, twice daily after meals, definitely with concomitant other anti-HIV agents. Strictly follow the instructions.
•Do not miss any single dose because you lose its effect if you miss a dose. If you miss a dose, take the missed dose as soon as possible. If it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. You should never take two doses at one time.
•If you accidentally take more than your prescribed dose, consult with your doctor or pharmacist.
•Do not stop taking this medicine unless your doctor instructs you to do so.
Precautions while taking this medicine
•Avoid taking this medicine with food or beverage containing hypericum perforatum(Saint. John's wort). Because hypericum perforatum may decrease the blood concentration of this medicine and reduce the effect of it.
Possible adverse reactions to this medicine
The most commonly reported adverse reactions include rash, diarrhea, and nausea. If any of these symptoms occur, consult with your doctor or pharmacist.
The symptoms described below are rarely seen as initial symptoms of the adverse reactions indicated in brackets. If any of these symptoms occur, stop taking this medicine and see your doctor immediately.
•high fever (38 degrees or higher), red eyes, erosive mouth/lips, red rash [toxic epidermal necrolysis, mucocutaneous-ocular syndrome, polymorphic erythema, hypersensitivity reaction with the rash of the whole body symptom (including drug hypersensitivity syndrome)]
•loss of appetite, dullness, nausea [hepatitis]
•decreased urination output, swelling, headache [renal failure, acute renal failure]
•muscle pain of limbs, stiffness, numbness [rhabdomyolysis]
The above symptoms do not describe all the adverse reactions to this medicine. Consult with your doctor or pharmacist if you notice any symptoms of concern other than those listed above.
Storage conditions and other information
•Keep out of the reach of children. Store away from direct sunlight, heat and moisture.
•Discard the remainder. Do not store them. If you dispose of unused medicines, seek advice of your pharmacist or medical institutionabout proper disposal of drugs.
