Indications |
As a surface anaesth & lubricant for the male & female urethra during cystoscopy, catheterisation, exploration by sound & other endourethral procedures, nasal & pharyngeal cavities in endoscopic procedures eg gastroscopy & bronchoscopy, during proctoscopy & rectoscopy & tracheal intubation. Symptomatic treatment of pain in connection w/ cystitis & urethritis. Pain relief after circumcision in childn.

Dosage |
Urethral anaesth Men 20 mL. Apply a penile clamp for several min, then instill the rest of the jelly. Sounding or cytoscopy 30-40 mL in 3-4 portions for 10 min prior to instrument insertion. Women 5-10 mL in small portions to fill the whole urethra. Endoscopy 10-20 mL. Proctoscopy & rectoscopy Up to 20 mL for anal & rectal procedures. Max dose: 400 mg. Lubrication for endotracheal intubation 2 mL applied to the surface of the tube just prior to insertion. Childn <12 yr Max: 6 mg/kg.

Contraindications |
Known hypersensitivity to local anaesth of the amide-type, methyl-/propyl paraben or their metabolite PABA. Formulations of lidocaine-containing parabens should be avoided in patients allergic to ester local anaesth.

Special Precautions |
Avoid excessive or prolonged use. Patients w/ traumatized mucosa &/or sepsis in the region of the proposed application. When used in the mouth, pharynx or throat area, may impair swallowing & thus enhance the danger of aspiration. Numbness of the tongue or buccal mucosa may increase the danger of biting trauma. Avoid introduction of jelly into the lumen to prevent occlusion. Patients treated w/ class III antiarrhythmic drugs eg amiodarone. Patients w/ partial or complete heart block or sepsis, deliberated or elderly patients, childn >12 yr, acutely ill patients or patients w/ advanced liver disease or severe renal dysfunction. May temporarily impair locomotion & coordination. Xylocaine jelly 20 mg/mL is probably porphyrinogenic & should only be prescribed to patients w/ acute porphyria on strong or urgent indications. Appropriate precautions should be taken for all porphyric patients.

Adverse Drug Reactions |
Rarely, anaphylactic shock. Post-op sore throat following endotracheal tube lubrication. Anaphylactic shock caused by other constituents. Acute systemic toxicity if overdosed.

Drug Interactions |
Additional systemic toxicity when used concomitantly w/ other local anaesth or agents structurally related to local anaesth eg tocainide. Cimetidine & β-blockers may cause potential toxic plasma conc when lidocaine is given in high dose for long-term use.

Pregnancy Category (US FDA) |
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
