Indications |
Production of local or regional anaesth by infiltration techniques; IV regional anaesth, by peripheral nerve block techniques eg intercostal blocks; major plexus blocks eg brachial plexus blocks & by epidural & subarachnoid blocks.

Dosage |
The lowest dosage that results in effective anaesth should be used. Adult Infiltration 20 mL of 1% inj or 10 mL of 2% inj. Paravertebral nerve block 3-5 mL of 1% inj. Pudendal block 10 mL of 1% inj on each side. Paracervical block 10 mL of 1% inj. Stellate ganglion block Cervical: 5 mL of 1% inj. Lumbar: 10 mL of 1% inj. Epidural anaesth Thoracic: 10-20 mL of 1% inj. Lumbar: 5-10 mL of 2% inj. Lumbar or caudal epidural analgesia 10-20 mL of 1% inj.

Overdosage |

Contraindications |
Known hypersensitivity to local anaesth of the amide-type, or methyl parahydroxybenzoate (MDV only). Not to be used for epidural or spinal anaesth in patients w/ uncorrected hypotension or w/ coagulation disorders or receiving anticoagulation treatment. Not to be used when there is inflammation &/or sepsis in the region of proposed inj or in the presence of septicaemia.

Special Precautions |
Inj should always be made slowly w/ frequent aspirations. Debilitated, elderly or acutely ill patients. Patients w/ preexisting abnormal neurological conditions, severe bradycardia, cardiac conduction disturbances or severe digitalis intoxication, hepatic &/or renal impairment, genetic predisposition to malignant hyperthermia. Paracervical blocks during pregnancy. Avoid intravascular inj. Multiple-dose vials are not recommended for intrathecal anaesth.

Adverse Drug Reactions |
CNS reactions, either excitatory or depressant. CV reactions, mainly depressant. Maternal hypotension. Rarely allergic reactions.

Drug Interactions |
Antiarrhythmic drugs, β-blockers, cimetidine, anticonvulsive agents eg phenobarb, primidone, carbamazepine & diphenylhydantoin, amiodarone, inhalational anaesth, skeletal muscle relaxants, structurally-related local anaesth.

Pregnancy Category (US FDA) |
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
