抗癌药Torisel（Temsirolimus，中文译名：西罗莫司）注射液由原惠氏制药公司研发（现由辉瑞公司生产），于2007年5月由美国FDA批准上市，用于治疗肾细胞癌。该药是第一款作用于mTOR的抗肿瘤药物。肾细胞癌是一种常见的癌症，约占所有癌症的2-3%，并且晚期肾细胞癌的5年生存率只有5-10%。

作用机理
Temsirolimus是mTOR（雷帕霉素的哺乳动物靶标）的抑制剂。temsirolimus与细胞内蛋白质（FKBP-12）结合，并且该蛋白质-药物复合物抑制了控制细胞分裂的mTOR活性。抑制mTOR活性导致在治疗的肿瘤细胞中G1生长停滞。当mTOR被抑制时，其磷酸化PI3激酶/AKT途径中mTOR下游的p70S6k和S6核糖体蛋白的能力被阻断。在使用肾细胞癌细胞系的体外研究中，temsirolimus抑制mTOR的活性，并导致缺氧诱导因子HIF-1和HIF-2α以及血管内皮生长因子的水平降低。
适应症和用途

剂量和给药
TORISEL的推荐剂量为25毫克，每周30-60分钟内静脉输注。治疗直至疾病进展或不可接受的毒性。

禁忌症
胆红素>1.5×ULN的患者禁用TORISEL。
警告和注意事项
超敏/输液反应（包括一些危及生命和罕见的致命反应）可能会在第一次输注TORISEL的早期发生。在整个输注过程中应监测患者。
要治疗超敏反应，请停止TORISEL并使用抗组胺药治疗。医生可以酌情以较慢的速度重新开始TORISEL。
肝功能不全：治疗轻度肝功能不全的患者时要谨慎并减少剂量。
高血糖和高血脂症很可能并且可能需要治疗。监测葡萄糖和脂质分布。
免疫抑制可能导致感染。
监测间质性肺疾病（ILD）的症状或影像学变化。如果怀疑是ILD，请停止使用TORISEL，并考虑使用皮质类固醇和/或抗生素。
可能会出现肠穿孔。立即评估发烧，腹痛，大便带血和/或急腹症。
发生肾衰竭，有时致命。在基线和TORISEL期间监测肾功能。
由于伤口愈合不正常，围手术期请谨慎使用TORISEL。
可能发生蛋白尿和肾病综合征。在开始TORISEL治疗之前和之后定期监测尿蛋白。患有肾病综合征的患者中止TORISEL。
应避免进行活疫苗接种和与接受活疫苗的人密切接触。

不良反应

药物相互作用

Torisel(temsirolimus)is a cancer medicine that interferes with the growth and spread of cancer cells in the body.
Torisel (temsirolimus) is a derivative of rapamycin.It is a drug with antifungal, immunosuppressive and antitumor activity.Torisel seems to prevent the action of mTOR, an enzyme that plays an important role in regulating protein synthesis that controls cell division. Therefore, Torisel may stop producing proteins that are essential for cancer cell proliferation.
Torisel is supplied as a 25mg/mL injection solution, designed for injection with 1.8mL of diluent. The recommended initial dose of the drug is 25mg once a week for 30-60minutes. Treat until disease progression or unacceptable toxicity. Before diluting the resulting solution with 250mL of 0.9% sodium chloride injection, the contents of the multi-hole injection bottle must first be diluted with a closed dilution.About 30minutes before the start of each dose, patients should receive prophylactic intravenous diphenhydramine 25to 50 mg (or similar antihistamines). The use of strong CYP3A4 inhibitors should be avoided. If co-administered, you should consider reducing the Torisel dose to 12.5mg/week. If the strong inhibitor is discontinued, the strong CYP3A4 inhibitor should be activated during the wash period approximately one week before allowing the Torisel dose to be adjusted to the previously used dose.In addition, the use of strong CYP3A4 inducers should be avoided.If co-administered, the torisel dose should be considered increased from 25mg/week to 50mg/week.If the strong inducer is stopped, the Torisel dose should be returned to the dose used before starting the strong CYP3A4 inducer.
