 Docetaxel（多西紫杉醇 Docetaxel，도세탁셀무수물）是一种处方化学疗法药物，用于治疗各种癌症，包括乳腺癌，前列腺癌，胃癌，头颈癌和非小细胞肺癌。
药理作用
属于紫杉类化合物抗肿瘤药。
本品的作用机制是加强微管蛋白聚合作用和抑制微管解聚作用，导致形成稳定的非功能性微管束，因而破坏肿瘤细胞的有丝分裂。本品在细胞内浓度比紫杉醇高3倍，并在细胞内滞留时间长,这是本品在体外试验中比紫杉醇抗肿瘤活性大的重要原因。在体内试验中，对小鼠的结肠癌、乳腺癌、肺癌、卵巢肿瘤移植物等有效。对顺铂、足叶乙苷、5Fu、或紫杉醇耐药的细胞株，本品不产生交叉耐药。　
适应证
对晚期乳腺癌、卵巢癌、非小细胞肺癌有较好的疗效。对头颈部癌、胰腺癌、小细胞肺癌、胃癌、黑色素瘤、软组织肉瘤也有一定的疗效。
用法与用量
静滴：每次135mg/m2或175mg/m2，每3周1次。用于乳腺癌，每次175mg/m，每3周1次。用于非小细胞肺癌，每次175～200mg/m，每3周1次。也可用于乳腺癌淋巴结阳性患者的辅助治疗。

Dosage
Breast cancer
Locally advanced or metastatic breast cancer
(1) When used in combination with doxorubicin(50mg/m2)in the first line therapy for breast cancer, 75mg/m2 is administered as docetaxel.
(2) When used as a combination therapy with trastuzumab for HER2 overexpression metastatic breast cancer that has never been treated with chemotherapy, 100mg/m2 as this drug is given intravenous instillation over 60 minutes every 3 weeks. You can start taking THIS DRUG the day after the first dose of trastuzumab, and if the tolerability of the previous dose of trastuzumab is good, you may start taking THIS DRUG immediately after taking the trastuzumab.
*Trastuzumab is an initial dose of 4 mg per kilogram of body weight for 90 minutes, followed by intravenous instillation of 2 mg per kilogram of body weight every week from the following week (refer to the usage and dosage of trastuzumab products).
(3) In breast cancer that has failed previous chemotherapy, this drug is administered intravenously for 1 hour every 3 weeks with 75-100mg/㎡.
(4) In case of locally advanced breast cancer or metastatic breast cancer that failed chemotherapy including anthracycline drugs in combination with caffecitabine, capecitabine was administered twice a day for 2 weeks while administering this drug 75mg/㎡ every 3 weeks, 1 It is recommended to administer 1,250mg/m2 once (within 30 minutes after meals) and take a break for 1 week (every 3 weeks). The dose of capecitabine is determined according to the body surface area by referring to the product manual of capecitabine.
· Adjuvant therapy after surgery
(1) Doxorubicin 50mg/m² and cyclophosphamide 500mg/m² every 3 weeks as an adjuvant therapy after surgery in breast cancer patients who can operate with positive and negative lymph nodes in combination with doxorubicin and cyclophosphamide. /M2 is administered for 6 cycles (TAC regimen). To reduce the risk of hematologic toxicity, G-CSF can be administered prophylactically (see Dosage Control section).
(2) After use of doxorubicin and cyclophosphamide, in combination with trastuzumab, postoperative adjuvant therapy (AC-TH regimen) for patients with operable breast cancer with overexpression of HER2
(a) AC: Doxorubicin (A) 60 mg/m 2 is administered every 3 weeks for 1 to 4 cycles (3 weeks/1 cycle), and cyclophosphamide (C) 600 mg/m 2 is administered.
(b) TH: Docetaxel (T) 100>mg/m² is administered for 1 hour every 3 weeks for 5 to 8 cycles (3 weeks/1 cycle), and trastuzumab (H) is administered weekly on the following schedule. do.
● Trastuzumab: Trastuzumab 4 mg/kg is administered for 90 minutes as an initial loading dose on Day 1 of the 5th cycle. After that, 2 mg/kg is administered for 30 minutes as a maintenance dose every 1 week (from the 8th day of the 5th cycle to the 8th cycle).
● Docetaxel: It is administered on the 2nd day of the 5th cycle. It is administered on the first day in 6-8 cycles.
(c) After the completion of adjuvant chemotherapy, 6 mg/kg of trastuzumab is administered every 3 weeks for 30 minutes. Trastuzumab is administered for a total of 1 year.
(3) TCH regimen (TCH regimen) (cycles 1-6) for patients with operable breast cancer with overexpression of HER2 in combination with carboplatin and trastuzumab
(a) Docetaxel and Carboplatin: Docetaxel (T) 75mg/m² every 3 weeks for 1 hour and Carboplatin (C) AUC 6 mg/mL/min for 30-60 minutes, 2 in 1 cycle It is administered on the first day, and on the first day in the 2nd to 6th cycle.
(b) Trastuzumab: 4 mg/kg of trastuzumab is administered as an initial loading dose on the first day for 90 minutes. After that, 2 mg/kg is administered every 1 week as a maintenance dose for 30 minutes.
(c) After the completion of adjuvant chemotherapy, 6 mg/kg of trastuzumab is administered every 3 weeks for 30 minutes. Trastuzumab is administered for a total of 1 year.
(4) In the postoperative adjuvant therapy (TC regimen) for breast cancer patients who can operate in combination with cyclophosphamide, docetaxel 75mg/m2 is injected for at least 1 hour on the first day of each cycle for 4 cycles with 3 weeks as 1 cycle. Then, 600mg/m2 of cyclophosphamide is injected intravenously for 30 to 60 minutes. One day before docetaxel administration, oral dexamethasone 8mg begins twice a day as a pretreatment, and continues to be taken for a total of five times.
