適應症

Local anesth.

用量

Max recommended single dose of lidocaine is 3 mg/kg (plain soln) or 7 mg/kg (soln with adrenaline). Adult, 70 kg Infiltration 20 mL of 1% inj or 10 mL of 2% inj. Paravertebral nerve block 3-5 mL of 1% inj. Pudendal block 10 mL of 1% inj on each side. Paracervical block 10 mL of 1% inj. Stellate ganglion block Cervical: 5 mL of 1% inj. Lumbar: 10 mL of 1% inj. Epidural anesth Thoracic: 10-20 mL of 1% inj. Lumbar: 5-10 mL of 2% inj. Lumbar or caudal epidural analgesia 10-20 mL of 1% inj.

As LA & cardiac drug.

Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1^st trimester (and there is no evidence of a risk in later trimesters).

禁忌

Hypersensitivity to amide local anesth. Epidural or spinal anesth in patients with uncorrected hypotension or coagulation disorders or receiving anti-coagulation treatment; inflammation &/or sepsis in the region of proposed inj or in the presence of septicemia; general contraindications related to epidural anesth should also be taken into account.

注意事項

Inj should always be made slowly with frequent aspirations. Careful & constant monitoring of cardiovascular & respiratory vital signs & the patient's state of consciousness. Patients anticoagulated or scheduled to be anticoagulated with LMW heparins or heparinoids are at risk of developing an epidural or spinal hematoma. Patients should be frequently monitored for signs & symptoms of neurological impairment. Elderly, young or debilitated patients, including those with advanced liver disease or severe renal dysfunction. Epilepsy, impaired cardiac conduction, bradycardia, severe shock or digitalis intoxication. Stokes-Adams syndrome or Wolff-Parkinson-White syndrome. CV impairment. Preexisting abnormal neurological pathology eg, myasthenia gravis. Use with extreme caution in epidural, caudal & spinal anesth when there are serious diseases of the CNS or of the spinal cord eg, meningitis, spinal fluid block, cranial or spinal hemorrhage, tumours, poliomyelitis, syphilis, TB or metastatic lesions of the spinal cord. Hepatic &/or renal impairment. Inadvertent IV or subarachnoid injection of small doses of local anesth injected into the head & neck area including retrobulbar, dental & stellate ganglion blocks, may produce adverse reactions. Prior to retrobulbar block, necessary equipment, drugs & personnel should be immediately available. Careful monitoring of the foetal heart rate is necessary. Patients receiving class III anti-arrhythmics. May impair ability to drive or operate machinery.

不良反應

CNS excitation or depression; bradycardia, hypotension & CV collapse; HTN & cardiac arrhythmias; methemoglobinaemia; maternal hypotension; allergic reactions; neurologic reactions eg persistent numbness, paresthesia & other sensory disturbances. Positional headache, hypotension, backache & shivering. Spinal block, loss of bladder & bowel control. Loss of perineal sensation & sexual function. Peripheral nerve injury & arachnoiditis.

交互作用

Antiarrhythmic drugs (disopyramide, procainamide, mexiletine, amiodarone), β-blockers, cimetidine, phenobarbital, primidone, carbamazepine & phenytoin, inhalational anesth, skeletal muscle relaxants, structurally-related local anesth.
