Contents Rifampicin 150 mg, INH 75 mg, pyrazinamide (PZA) 400 mg, ethambutol 275 mg
Indications Initial & re-treatment phase of pulmonary & extrapulmonary TB in adults.
Dosage 2-mth initial phase Adult & childn ≥13 yr, ≥71 kg 5 tab daily, 55-70 kg 4 tab daily, 38-54 kg 3 tab daily, 30-37 kg 2 tab daily.
Administration Should be taken on an empty stomach (Take w/ a full glass of water 1 hr before or 2 hr after meals. May be taken w/ meals if GI irritation occurs.).
Contraindications Hypersensitivity. Jaundice or preexisting liver disorders, optic neuritis. Childn <13 yr. Pregnancy & lactation.
Special Precautions Impaired kidney & hepatic function, visual defects, history of gout & psychosis, neuropathy or pyridoxine deficiency, patients with epilepsy, porphyria & diabetes, elderly.
Adverse Drug Reactions GI disturbances, hypersensitivity reactions, neurologic effects. Rifampicin: Cutaneous reactions, pseudomembranous colitis, hepatitis, changes in renal & hepatic function, hematological effects, orange-red discoloration urine & other body fluids. INH: Peripheral neuropathy, hematological effects. PZA: Hepatotoxicity, hyperuricemia. Ethambutol: Retrobulbar neuritis, reduced renal clearance of urate.

Drug Interactions Rifampicin: Induces microsomal enzymes (accelerates clearance of methadone, oral anticoagulants, glucocorticoids, oestrogen, oral hypoglycaemic agents, digitoxin, antiarrhythmics, theophylline, anticonvulsants, β-blockers, azole antifungal, cyclosporin). INH: Inhibits metabolism of antiepileptics, benzodiazepines, warfarin, theophylline. Increases metabolism of enflurane. Alcohol, ASA, Al-containing antacids, corticosteroids, ketoconazole, propranolol. PZA: Decreases efficacy of gout therapy agents. Ethambutol: Neurotoxic agents.
