Indications |
Essential HTN; CHF; reduction of the risk of stroke recurrence when used in combination w/ indapamide in patients w/ history of stroke or TIA; reduction of CV events in patients w/ stable angina when used w/ other preventive meds.

Dosage |
HTN 4 mg as single dose, may be increased to a single 8 mg dose. Elderly Start treatment w/ a 2 mg dose. In renal insufficiency, dose has to be adapted according to creatinine clearance. CHF Single oral dose of 2 mg, may be increased to a single 4 mg dose once BP acceptability has been demonstrated. Reduction of risk of stroke recurrence Initial dose 2 mg daily for 2 wk, then increased to 4 mg daily for another 2 wk before introducing indapamide. Reduction of risk of CV event Initially 4 mg daily for 2 wk, then increased to 8 mg depending on renal function. Elderly 2 mg daily for 1 wk, then 4 mg daily the next wk before increasing to 8 mg daily depending on renal function.

Special Precautions |
Arterial hypotension, renal failure, water & Na depletion; renovascular HTN; surgery/anesth; elderly. Hemodialysis: avoid combination w/ highly permeable membranes (polyacrylonitrile). Measure plasma creatinine at baseline & during the mth following institution of treatment. May precipitate excessive hypotension in severe CHF w/ or w/o associated renal insufficiency. Patients w/ ischaemic heart disease or cerebrovascular disease in whom excessive fall in BP may result in MI or CVA.

Adverse Drug Reactions |
GI disorders; dizziness, headaches, mood &/or sleep disorders, asthenia; taste disorder; cramps, localized rash; dry cough; angioneurotic edema.

Drug Interactions |
K supplements or K-sparing diuretics may induce hyperkalemia esp in renal failure. Diuretics. Neuroleptics or TCAs increase risk of orthostatic hypotension. Serum lithium conc may rise during lithium therapy.
