Indications |
Treatment of symptoms of depression including depression accompanied by symptoms of anxiety, in patients w/ or w/o history of mania; obsessive-compulsive disorder (OCD), panic disorder w/ or w/o agoraphobia, post-traumatic stress disorder (PTSD), social phobia, premenstrual dysphoric disorder (PMDD).

Dosage |
Depression & OCD 50 mg/day. Max: 200 mg/day. Panic disorder, PTSD & social phobia 25 mg/day, may be increased to 50 mg/day after 1 wk. Max: 200 mg/day. PMDD 50 mg/day, either daily throughout the menstrual cycle or limited to the luteal phase of menstrual cycle. Dose changes should be made at 1-wk intervals. Max: 150 mg/day. OCD Childn 13-17 yr Initially 50 mg/day, 6-12 yr Initially 25 mg/day increasing to 50 mg/day after 1 wk. Max: 200 mg/day. Administer once daily either in the morning or evening.

Special Precautions |
Activation of hypomania or mania. Unstable epilepsy, history of bleeding disorders, hyponatremia, hepatic & renal impairment. Monitor for signs & symptoms of serotonin syndrome or neuroleptic malignant syndrome. Concomitant use of drugs that affect platelet function. May impair ability to drive or operate machinery. Pregnancy & lactation.

Adverse Drug Reactions |
Diarrhoea/loose stools, dry mouth, dyspepsia, nausea, anorexia, dizziness, somnolence, tremor, insomnia, sexual dysfunction (principally ejaculatory delay in males), increased sweating.

Tryptophan, fenfluramine, 5-HT agonists, St John's wort, alcohol, lithium, phenytoin, sumatriptan, protein bound drugs, warfarin, cimetidine.

Pregnancy Category (US FDA) |
Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
