Dosage |
Major depressive episodes, obsessive-compulsive disorder 10 mg once daily. May be increased to 20 mg. Panic disorder w/ or w/o agoraphobia Initially 5 mg on the 1st wk. Then increase dose to 10 mg up to 20 mg daily. Social anxiety disorder 10 mg once daily. May be increased to 20 mg. Generalised anxiety disorder 10 mg once daily. Max (for all indications): 20 mg/day. Elderly >65 yr Initially ½ the usual recommended dose. Consider lower max dose. Severely reduced renal function (CrCl <30 mL/min) Adjust dose. Reduced hepatic function/poor CYP2C19 metabolisers Initially 5 mg daily for the 1st 2 wk. Max: 10 mg daily.

Special Precautions |
Avoid combination w/ moclobemide or serotonergic compd. Diabetics, history of mania/hypomania & co-administration of electroconvulsive therapy (ECT) in patients on SSRIs. Monitor patients w/ epilepsy esp unstable epilepsy. Discontinue if patient develops serotonin syndrome. Concomitant medicines that affect blood clotting, platelet function or patients w/ coagulopathy. Avoid abrupt w/drawal. Monitor patients for suicide risk in 1st few wk of treatment. Severe renal dysfunction (CrCl <30 mL/min).

Adverse Drug Reactions |
Dizziness, headache, nausea, sweating, somnolence, insomnia, constipation, diarrhoea, decreased or increased appetite, sexual dysfunction, fatigue, pyrexia, sinusitis, yawning.

Drug Interactions |
MAOI, selegiline, lithium, tryptophan, agents that may lower seizure threshold, St. John's Wort, omeprazole, high doses of cimetidine, drugs metabolised by CYP2D6, CYP2C19 enzymes.

Pregnancy Category (US FDA) |
Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus. |
