Metligine tables 500 × 2mg (midodrine hydrochloride midodrine tablets) for the treatment of postural Hypotension (orthostatic Hypotension)
Postural Hypotension (orthostatic Hypotension): refers to the decrease of systolic blood pressure ≥ 20mmHg and (or) the decrease of diastolic blood pressure≥10mmHg when changing from lying position to upright position (or head tilt more than 60 °). Common symptoms include positional dizziness, blackness, and falls with or without syncope; Less common symptoms include orthostatic cognitive dysfunction (patients with autonomic dysfunction may have significant executive dysfunction due to transient frontal lobe hypoperfusion when in orthostatic position), mental retardation, physical weakness Neck pain or discomfort under the occipital bone and beside the neck ("coat hanger" distribution - Hypotension induces ischemia of cervical banding muscles) or recumbent breathing (orthostatic Hypotension causes insufficient or disordered pulmonary perfusion during ventilation, resulting in dyspnea in the upright position).
Neurogenic orthostatic Hypotension is a subtype of orthostatic Hypotension. In addition to meeting the definition of orthostatic Hypotension, patients with neurogenic orthostatic Hypotension also have Autonomic nervous system damage, which is characterized by the inability to provide adequate autonomic postural response. The most significant role of autonomic nervous system is to maintain normal blood pressure through systematic Vasoconstriction and compensatory heart rate acceleration, This loss is largely due to insufficient Norepinephrine release from the Sympathetic nervous system nerve.
Elderly patients (especially those with systolic hypertension), diabetes patients, insufficient blood volume, decreased sensitivity of vascular baroreceptors, central Neuromodulation dysfunction and the use of vasodilators will increase the risk of postural Hypotension.
