药理作用
本品属钙离子内流阻滞剂，阻滞肌细胞的细胞膜对钙离子的内流。正常情况下，钙是在细胞内肌浆网释放，结合细胞外的钙离子内流，导致钙与调钙素结合的提高，调钙素与钙的复合物通过肌浆球蛋白氢链的磷酸化作用使平滑肌收缩。钙通道阻滞剂如伊拉地平通过抑制这两个过程，使小动脉扩张。此作用在血管平滑肌比心脏平滑肌明显。本品对心肌细胞作用较小，可使冠状血管、脑血管和骨骼肌血管床扩张，但它对心脏收缩力影响较小。本品减慢窦房结传导的作用和硫氮?酮相似，但对房室结没有影响。
药动学
口服吸收，肝脏首关代谢(首过代谢)82%，达峰时间2～3h。血浆半衰期8.8±7.1h，血浆蛋白结合率＞96%，表观分布容积283L/kg，表明在体内有蓄积。代谢产物约60%～65%通过尿液排出，其余由粪便排出。
适应证
高血压、冠状粥样硬化性心脏病心绞痛。
禁忌证
无绝对禁忌证。
注意事项
孕妇及哺乳期妇女，有窦房结病变患者慎用。
不良反应
不良反应与其他扩血管药相似，有头痛、面部潮红、头晕、心动过速、心悸、局部周围水肿，少见的有低血压，增加体重、乏力、腹部不适和皮疹等。
用法用量
1.高血压：初始剂量2.5mg，每天2次。4周后血压控制不满意，可加量至5mg，每天2次。
2.心绞痛：可用于慢性、稳定型、运动诱发性心绞痛的预防性用药。
药物相应作用
与普萘洛尔合用，可增加普萘洛尔的生物利用度，增加降压效果。126.com与ACEⅠ合用，可增加降压作用。
