吉利德公司近日宣布，日本厚生劳动省批准Vemlidy(替诺福韦艾拉酚胺，TAF ベムリディ錠)25mg每日1次用于抑制乙肝病毒复制。
TAF是另一款常用乙肝药物Viread(替诺福韦二吡呋酯，TDF)的前药，因为TAF具有较高的血液稳定性，可以有效递达至肝细胞，因此TAF在剂量低于Viread十分之一(25mg/300mg)的情况下就能发挥与后者相似的疗效，还可避免血液中替诺福韦浓度过高，提高了安全性。
TAF在日本获批主要基于两项为期48周的Ⅲ期、非劣效研究(Study 108和Study 110)的数据。
Study 108研究入组了425例乙肝e抗原(HBeAg)阴性患者，按2：1分组随机给予Vemlidy或Viread，治疗48周，其中27例患者来日日本的11家中心。Study 110研究入组了873例HBeAg阳性患者，按2：1分组随机给予Vemlidy或Viread，治疗48周。其中46例患者来自日本的16家中心。研究的主要终点是48周时HBV DNA水平低于29IU/mL的患者比例。
结果显示，Study 108研究中，TAF治疗组HBV DNA水平低于29IU/mL的患者比例为94%(268/285)，TDF组为92.9%(130/140)，到达了非劣效终点。
Study 110研究中，TAF治疗组HBV DNA水平低于29IU/mL的患者比例为63.9%(371/581)，TDF组为66.8%(195/292)，达到了非劣效终点。
Gilead首席科学官及执行副总裁Norbert Bischofberger表示：“日本目前有超过100万例乙肝患者，我们相信Vemlidy可以为这类患者群体提供一种新的治疗选择。”
11月10日，FDA批准了Vemlidy(替诺福韦艾拉酚胺，TAF)25mg每日1次用于治疗伴有代偿性肝病的慢性乙肝病毒(HBV)感染患者。
11月11日，欧盟人用药品委员会(CHMP)对Vemlidy的上市申请做出肯定推荐，也建议批准TAF 25mg每日1次用于初治或接受过治疗的成人及青少年(年龄≥12岁，体重≥35kg)慢性乙肝感染，意味着欧盟不久后也将正式批准Vemlidy。
