Fludarabine phosphate

適應症

Initial treatment of patients with β-cell chronic lymphocytic leukemia (CLL) & low grade non-Hodgkin's lymphoma for patients who have not responded to or whose disease has progressed during or after treatment with at least 1 standard alkylating agent-containing regimen. First-line treatment should only be initiated in patients with advanced disease, RAI stages III/IV (Binet stage C) or RAI stages I/II (Binet stage A/B), where the patient has disease-related symptoms or evidence of progressive disease.

用量

Tab Adult 40 mg/m² body surface given daily for 5 consecutive days every 28 days. Vial Adult 25 mg/m² body surface given daily for 5 consecutive days every 28 days by IV infusion over 30 mins.

過量

View Fludara[福達樂] overdosage for action to be taken in the event of an overdose.

May be taken with or without food (Swallow whole, do not break/chew/crush.).

Category D: There is positive evidence of human foetal risk, but the benefits from use in pregnant women may be acceptable despite the risk (e.g., if the drug is needed in a life-threatening situation or for a serious disease for which safer drugs cannot be used or are ineffective).

禁忌

Renal impairment with CrCl < 30 mL/min; decompensated hemolytic anemia; pregnancy & lactation.

注意事項

Severe bone marrow suppression (notably anemia, thrombocytopenia & neutropenia) or myelosuppression may occur. Closely observe patients for signs of hematologic & non-hematologic toxicity. Periodic assessment of peripheral blood counts recommended to detect development of anemia, neutropenia & thrombocytopenia. Tumor lysis syndrome. Monitor for signs of autoimmune hemolytic anemia. Measure CrCl for renal-impaired patients & those > 70 years. Elderly. Females of childbearing potential or males should take contraceptive measures for at least 6 months after stopping therapy. Avoid live vaccines during & after treatment. Patients who require blood transfusion during therapy should receive use irradiated blood only.

不良反應

Myelosuppression (neutropenia, thrombocytopenia, anemia), visual disturbances, nausea, vomiting, diarrhea, mucositis, anorexia, stomatitis, fever, chills, fatigue, weakness, malaise, infections, edema, peripheral neuropathy, pneumonia, rash. Rarely, autoimmune phenomena (e.g. autoimmune hemolytic anemia, thrombocytopenic purpura, pemphigus, Evan's syndrome), tumor lysis syndrome, neurotoxicity.

交互作用

Concomitant use with pentostatin not recommended. Effectiveness may be reduced by dipyridamole & other inhibitors of adenosine uptake.
