適應症 |
Monotherapy in adult & children >12 year or as add-on therapy in adult & children > 2 year for the treatment of epilepsy (partial seizures & generalised seizures), including tonic-clonic seizures & seizures associated with Lennox-Gastaut syndrome. Prevention of depressive episodes in patients with bipolar disorder. |

用量 |
Monotherapy Adult & children >12 years Initially 25 mg once daily for 2 weeks, then 50 mg once daily for 2 weeks. Maintenance: 100-200 mg daily in 1-2 divided doses. Add on therapy Adult & children >12 years Patients taking valproate with or without any other antiepileptic drug (AED) Initially 25 mg every alternate day for 2 weeks, then 25 mg once daily for 2 weeks, followed by max. increments of 25-50 mg per 1-2 weeks. Maintenance: 100-200 mg/day in 1-2 divided doses. Patients not taking valproate but taking concomitant AEDs or other medications that induce lamotrigine glucuronidation with or without other AEDs Initially 50 mg once daily for 2 weeks, then 100 mg/day in 2 divided doses for 2 weeks, followed by max.increment of 100 mg per 1-2 weeks. Maintenance: 200-400 mg/day in 2 divided doses. Patients taking oxcarbazepine without any other inducers or inhibitors of lamotrigine glucuronidation Initially 25 mg once daily for 2 weeks, then 50 mg once daily for 2 weeks, followed by max. increment of 50-100 mg per 1-2 weeks. Maintenance: 100-200 mg/day in 1-2 divided doses. Children 2-12 years Patients taking valproate with or without any other AEDs Initially 0.15 mg/kg once daily for 2 weeks, then 0.3 mg/kg once daily for 2 weeks, followed by max. increment of 0.3 mg/kg per 1-2 weeks. Maintenance: 1-5 mg/kg/day in 1-2 divided doses. Max: 200 mg/day. Patients not taking valproate but taking concomitant AEDs or other medications that induce lamotrigine glucuronidation with or without AEDs Initially 0.6 mg/kg/day in 2 divided doses for 2 weeks, then 1.2 mg/kg/day in 2 divided doses for 2 weeks, followed by max. increment of 1.2 mg/kg per 1-2 weeks. Maintenance: 5-15 mg/kg/day in 2 divided doses. Max: 400 mg/day. Patients taking oxcarbazepine without any other inducers or inhibitors of lamotrigine glucuronidation Initially 0.3 mg/kg/day in 1-2 divided doses for 2 weeks, then 0.6 mg/kg/day in 1-2 divided doses for 2 weeks, followed by max. increment of 0.6 mg/kg per 1-2 weeks. Maintenance: 1-10 mg/kg/day in 1-2 divided doses. Max: 200 mg/day. Bipolar disorder Adult & children >18 years Adjunct therapy with inhibitors of lamotrigine glucuronidation (eg valproate) Initially 25 mg every alternate days for 2 weeks, followed by 25 mg once daily for 2 weeks then 50 mg daily in 1-2 divided doses in week 5. Target stabilization dose: 100 mg daily in 1-2 divided doses. Max: 200 mg daily. Adjunct therapy with inducers of lamotrigine glucuronidation in patients not taking inhibitors of lamotrigine glucuronidation (eg valproate) Initially 50 mg once daily for 2 weeks, followed by 100 mg/day in 2 divided doses for 2 weeks then 200 mg/day in 2 divided doses in week 5. Target stabilization dose: 300 mg in 2 divided doses in week 6, increasing to 400 mg/day in 2 divided doses if necessary in week 7. Monotherapy or adjunct therapy with agents known not to induce or inhibit lamotrigine glucuronidation Initially 25 mg once daily for 2 weeks, followed by 50 mg daily in 1-2 divided doses for 2 weeks then 100 mg daily in 1-2 divided doses in week 5. Target stabilization dose: 200 mg (range: 100-400 mg) daily in 1-2 divided doses. |

用法 |
May be taken with or without food (Dispersible/chewable tab may be swallowed whole, chewed or dispersed in water/ diluted fruit juice. If chewed, consume a small amount of water/ diluted fruit juice to aid in swallowing. To disperse, add the tab to a small amount of liqd in a glass (1 tsp or enough to cover). When the tab is completely dispersed (approx 1 min), swirl the solution & consume the entire quantity immediately.). |

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus. |

注意事項 |
Closely monitor & consider withdrawal if hypersensitivity syndrome develops. Avoid abrupt w/drawal (gradual decrease over a period of 2 weeks). Monitor for worsening of depressive symptoms &/or the emergence of suicidal ideation/behaviour or thoughts of self-harm. Monitor patient with bipolar disorder for clinical worsening & suicidality. Report changes in menstrual pattern ie breakthrough bleeding. Renal failure. Pregnancy & lactation. May affect ability to drive or operate machinery. |

Skin rash, irritability, headache, drowsiness, insomnia, dizziness, tremor, nystagmus, ataxia, diplopia, blurred vision, nausea, vomiting, diarrhea, tiredness, arthralgia, pain, back pain.

交互作用 |
Valproate inhibits glucuronidation of lamotrigine. Carbamazepine, phenytoin, primidone, phenobarb, rifampicin, lopinavir/ritonavir, atazanavir/ritonavir, ethinyloestradiol/levonorgestrel combination induce glucuronidation of lamotrigine.
