適應症

Treatment of advanced ovarian cancer which has relapsed at least 6 months following platinum-based therapy. In combination with paclitaxel for 1st-line treatment of patients with metastatic breast cancer after failure of prior anthracycline-containing adjuvant chemotherapy, unless anthracyclines were clinically contraindicated. In combination with cisplatin for the 1st-line treatment of patients with inoperable, locally advanced (Stage IIIA or IIIB), or metastatic (Stage IV) non-small cell lung cancer. 1st-line treatment locally advanced (nonresectable Stage II or Stage III) or metastatic (Stage IV) adenocarcinoma of the pancreas in patients previously treated with 5-FU. In combination with cisplatin for 1st-line treatment for Stage IV (locally advanced or metastatic) Transitional Cell Carcinoma (TCC) of the urothelium in bladder cancer.

用量

Pancreatic cancer 1000 mg/m² by 30-min IV infusion once weekly for up to 7 weeks, followed by a week of rest. Subsequent cycles consist of infusions once weekly for 3 consecutive weeks out of every 4 weeks. NSCLC 1000 mg/m² by 30-min IV infusion on days 1, 8 & 15 of each 28-day cycle or 1250 mg/m² by 30-min IV infusion on days 1 & 8 of each 21-day cycle. Administer cisplatin at 100 mg/m² IV on day 1 after the infusion of gemcitabine. Bladder cancer 1000 mg/m² by 30-min IV infusion on days 1, 8, & 15 of each 28-day cycle. Administer cisplatin at 70 mg/m² IV on day 1 of each 28-day cycle. Breast cancer 1250 mg/m² by 30-min IV infusion on days 1 & 8 of each 21-day cycle. Administer paclitaxel at 175 mg/m² as a 3-hr IV infusion on day 1 before gemcitabine. Ovarian Cancer 1000 mg/m² by 30-min IV infusion on days 1 & 8 of each 21-day cycle. Carboplatin AUC 4 should be administered IV on day 1 after Gemzar administration.

過量

注意事項

Monitor for myelosuppression. Discontinue in severe lung toxicity. Laboratory eva luation of renal & hepatic function should be performed prior initiation of therapy & periodically thereafter. Lactation. Preexisting renal impairment or hepatic insufficiency. Concurrent liver metastates or history of hepatitis, alcoholism or liver cirrhosis. Initiate after acute effects of radiotherapy have resolved or at least 1 week after radiation.

不良反應

Myelosuppression with anemia, leukopenia or thrombocytopenia; nausea & vomiting, diarrhea & stomatitis; transient elevations in serum transminases; mild proteinuria & hematuria, hemolytic uremic syndrome (discontinue); fever & flu-like symptoms; rash, pruritus; dyspnea, occasionally accompanied by bronchospasm, pulmonary toxicity (discontinue); edema; infections, sepsis (rare); alopecia; paresthesia; inj site reactions; allergic reactions(rare); CV events (rare).
