藥理作用 
Galvus Met合併兩種在藥物作用機轉上具互補性的抗高血糖藥物，可改善第二型糖尿病患者的血糖控制。
Vildagliptin屬於胰島增強劑，而metformin hydrochloricde屬於biguanide類藥物。Vildagliptin屬於胰島增強劑，是一強效且選擇性的DPP-4抑制劑。
Metformin主要作用為降低內生性肝葡萄糖的產量。
適應症 
第二型糖尿病之治療:
- 適用於成年人配合飲食和運動，以改善下列第二型糖尿病患者的血糖控制：已接受vildagliptin 與metformin合併治療者，或單獨使用metformin 或vildagliptin，但血糖控制不佳者。
- 當飲食及運動加上sulphonylurea 及metformin雙重療法無法提供適當的血糖控制時，Galvus Met可與sulphonylurea 併用。
- 當飲食及運動加上穩定劑量的胰島素及metformin仍無法提供適當的血糖控制時，Galvus Met亦適合與胰島素合併使用。
用法用量 
成年人：
依患者目前使用的metformin 劑量，Galvus Met的起始劑量可能為每天兩次50毫克/500毫克、50毫克/850毫克或50毫克/1000毫克錠劑，分別於早晨與晚間服用。每日建議劑量為100毫克vildagliptin 加上2000 毫克metformin。
同時使用vildagliptin與metformin個別錠劑的患者，可轉換至使用含有相同成分劑量的Galvus Met。Vildagliptin每日總劑量不超過100毫克。
藥動力學 
Vildagliptin
吸收
空腹口服vildagliptin吸收快速，於1.7小時達到最高血漿濃度。
分佈
Vildagliptin 與血漿蛋白的結合率低(9.3%)，且在血漿與紅血球的分布相等。靜脈注射到達穩定狀態時的平均分佈體積(Vss)為71公升，顯示具血管外分佈。
代謝
代謝是vildagliptin於人體的主要排除路徑，佔劑量的69%。主要代謝物為LAY151，不具藥理活性，其為cyano部分的水解產物，佔劑量的57%。其次是amide 的水解產物 ( 佔劑量的4%)
排除
口服以[14C]標定的vildagliptin，約85%的劑量隨尿液排除，15%的劑量則排除於糞便中。口服後由腎臟排除的vildagliptin原型約佔23%之服用劑量。
Metformin
吸收
最大血漿濃度約發生於口服metformin後約2.5小時(Tmax)。500毫克metformin 錠劑在健康受試者中的絕對生體可用率約為50 ~ 60%。口服藥物後，在糞便中採集到的未吸收比例約為20至30%。
分佈
血漿蛋白結合率可忽略不計。Metformin會分佈至紅血球。平均分佈體積(Vd)介於63至276公升。
代謝
Metformin 以原型方式從尿液中排出。未在體內找到代謝物。
排泄
Metformin 由腎臟排除至體外。Metformin的腎臟清除率大於400毫升/分鐘，代表metformin可經由腎絲球過濾和腎小管分泌作用排除至體外。在口服劑量後，平均最終排除半衰期約為6.5 小時。
副作用 
低血糖、頭暈、腸胃不適。
交互作用 
需謹慎使用的複方製劑
糖皮質類固醇、beta-2-促進劑與利尿劑具有內生性升糖作用。病患應了解必要的用藥資訊，並應接受多次血糖監測，特別是開始治療時需確實監測。如有必要，可能需在併用藥物期間與停藥時調整Galvus Met的劑量。血管收縮素轉化(ACE)抑制劑可能會降低血糖濃度。如有必要，應在接受其他藥物治療期間與停藥時調整抗高血糖藥物的劑量。
禁忌 
1. 已知對vildagliptin-metformin或賦形劑之任何成分過敏者，禁止使用Galvus Met。
2. 糖尿病酮酸血症或糖尿病性的昏迷前狀態。
3. 腎衰竭或腎功能異常，定義為肌酸酐廓清率小於60毫升/分鐘。
4. 可能會影響腎功能的急性病症，例如：脫水、嚴重感染、休克、經血管注射含碘顯影劑。
5. 可能會導致組織缺氧的急性或慢性疾病，例如：心臟衰竭或呼吸衰竭、近期罹患的心肌梗塞、休克。
6. 肝功能不全。
7. 急性酒精中毒、酗酒。
8. 哺乳。
給付規定 
2.使用條件：
(1)原則上第二型糖尿病治療應優先使用metformin，或考慮早期開始使用胰島素。除有過敏、禁忌症、不能耐受或仍無法理想控制血糖的情形下，可使用其他類口服降血糖藥物。
(2)TZD製劑、DPP-4抑制劑、SGLT-2抑制劑、以及含該3類成分之複方製劑，限用於已接受過最大耐受劑量的metformin仍無法理想控制血糖之第二型糖尿病病人，且SGLT-2抑制劑與DPP-4抑制劑及其複方製劑宜二種擇一種使用。
(3)第二型糖尿病病人倘於使用三種口服降血糖藥物治療仍無法理想控制血糖者，宜考慮給予胰島素治療。
(4)特約醫療院所應加強衛教第二型糖尿病病人，鼓勵健康生活型態的飲食和運動，如控制肥胖、限制熱量攝取等措施。
(5)第二型糖尿病病人使用之口服降血糖藥物成分，以最多四種(含四種)為限。
備註：本規定生效前已使用超過四種口服降血糖藥物成分之病人，得繼續使用原藥物至醫師更新其處方內容。
5.1.4.vildagliptin
若與sulphonylurea合併使用時，vildagliptin每日建議劑量為50mg。
飲食提示 
不建議合併
使用由於Galvus Met含有metformin 藥物成分，急性酒精中毒者會有較高的風險出現乳酸中毒(尤其是空腹、營養不良或肝功能不全之情況) 。應避免飲酒及使用含有酒精成分的藥物。
警語 
Galvus Met不適用於第一型糖尿病患者。
過量處理 
清除metformin的最有效方法為血液透析。Vildagliptin則不能經由血液透析排除，但其主要水解代謝物(LAY 151)可藉由血液透析去除。建議給予支持性療法。
藥品保存方式 
30℃以下，避光儲存。
General Information on Galvus Met
Galvus Met is used for controlling high blood sugar levels in patients with non-insulin dependent Type 2 diabetes. It is a combination of two drugs: Metformin and Vildagliptin. The medicine is given to a patient along with an exercise program and diet plan. It is usually given to patients whose blood sugar levels cannot be controlled by giving Metformin or Vildagliptin separately.
Both Metformin Hydrochloride and Vildagliptin are antihyperglycemic agents. Metformin belongs to the class of biguanides, while Vildagliptin is a DPP-4 inhibitor. Vildagliptin works by boosting glycemic control in the patient’s body, while Metformin decreases the amount of sugar produced by the liver and absorbed by the stomach or intestines.
Galvus Met is available in the form of oral tablets. Controlling blood sugar levels helps in preventing the loss of limbs, nerve problems, blindness, kidney damage, or sexual problems associated with the condition. It may also decrease risks of a stroke or heart attack.
Side effects of Galvus Met
Patients with certain medical conditions may have a higher risk of suffering from the serious side effects of Galvus Met. Such medical conditions include hypersensitivity to Metformin, Vildagliptin or any other ingredients in the drug, renal disease, renal dysfunction, congestive heart failure, or diabetic ketoacidosis. Patients undergoing a radiologic study that involves iodinated contrast materials administered intravascularly will also be asked to discontinue taking the medicine temporarily. Do not buy Galvus Met if you are suffering from any of the above conditions.
As the drug causes blurred vision, drowsiness, or dizziness, you should not drive a vehicle, operate a machine, or do any such activity that requires complete alertness and clear vision. It may result in a serious accident.
Limit alcohol intake during Galvus Met treatment, as it may increase risks of developing lactic acidosis or low blood sugar. Symptoms of such a condition include dizziness, vomiting, stomach pain, nausea, and facial flushing.
Nausea, diarrhea, weight gain, or stomach upset may commonly occur in Galvus Met patients. However, if any of the stomach symptoms return after several days or weeks of starting the treatment, they may be signs of lactic acidosis, and you need to get it treated as soon as possible.
Some serious side effects associated with Galvus Met include easy bruising or bleeding, persistent nausea, signs of a new infection such as fever and persistent throat soreness, yellowing of the eyes or skin, severe stomach or abdominal pain, or dark urine. Inform your doctor immediately if you have any of these symptoms.
Dosage of Galvus Met
Galvus Met is available in two different strengths: 850mg Metformin and 50mg Vildagliptin, and 1000mg Metformin and 50mg Vildagliptin. Your doctor may ask you to take one tablet once or twice a day.
Drink lots of fluids during Galvus Met treatment. Try to take the tablet at almost the same time each day.
Drug Interactions of Galvus Met
Before you buy Galvus Met, inform your doctor if you are already taking azole antifungals (e.g. ketoconazole or fluconazole), water pills or diuretics (e.g. hydrochlorothiazide or furosemide), MAO inhibitors (e.g. linezolid, phenelzine or selegiline), or drugs that affect the ability of your kidneys to remove Metformin (e.g. cephalexin or cimetidine).
