简介：

近日，美国食品和药物管理局（FDA）已批准ZINPLAVA®（bezlotoxumab，贝佐洛单抗）注射液，减少18岁以上患者艰难梭菌感染（CDI）的复发。
人单克隆抗体特别适用于正在服用CDI抗生素并有再次感染风险的成年人。贝佐洛单抗本身不是抗生素，而是与毒素B结合并中和，毒素B是艰难梭菌产生的几种毒素之一，并且被认为是威胁细菌生命的致命毒素。
在1小时内以10 mg/kg的单次剂量静脉内（IV）施用贝洛酮单抗。
ZINPLAVA®可以减少正在接受CDI抗菌药物治疗且高CDI复发风险的18岁或18岁以上患者的艰难梭菌感染（CDI）复发。

作用机制
ZINPLAVA(bezlotoxumab)是一种人单克隆抗体结合至艰难梭菌毒素B和中和其效应[见微生物学]。
适应证和用途
ZINPLAVA是一种人单克隆抗体结合至艰难梭菌Clostridium difficile毒素B，适用于减低艰难梭菌感染的复发(CDI)在患者18岁或以上患者正在接受CDI的抗细菌治疗和是处于对CDI复发高风险。
使用的限制：
ZINPLAVA是不适用为CDI的治疗。ZINPLAVA不是一种抗细菌药。ZINPLAVA只应被与CDI的抗细菌治疗结合使用。
剂量和给药方法
●为CDI抗细菌药治疗期间给予ZINPLAVA。
● 推荐剂量是单剂量10mg/kg历时60 分钟静脉输注给予。
● 静脉输注前稀释。通过一个低-蛋白结合0.2微米至5微米在线或添加滤器给药。对稀释和给药指导见完整处方资料。

警告和注意事项
心力衰竭：在两项3期临床试验中有充血性心衰史(CHF) ZINPLAVA-治疗患者报道更常见。在有CHF史患者，ZINPLAVA应保留当获益胜过风险时使用。
不良反应
最常见不良反应(报道在≥4%患者)包括恶心，发热，和头痛。
提供/存储和处理
ZINPLAVA注射液：无菌，无防腐剂，透明至中等乳白色，无色至淡黄色溶液，并以以下包装配置提供：
纸盒（NDC 0006-3025-00）含有一（1）个ZINPLAVA单剂量小瓶1,000mg/ 40mL（25mg/mL）。

Recently, Merck’s drug ZINPLAVA(bezlotoxumab)was approved by the FDA to reduce the risk of recurrence of Clostridium difficile.
Clostridium difficile infection is common in hospitalized patients who have recently used certain antibiotics or other drugs. Clostridium difficile produces toxin B after infecting patients, causing mild to severe diarrhea, abdominal pain, and fever. Recurrent C. difficile infections has always been a challenge in this field. About a quarter of patients will experience relapses after the first infection, and more than 40% of patients will experience multiple relapses. Since the recurrence of Clostridium difficile is more common in people over 65 years old and people with impaired immune systems, these symptoms will greatly affect their lives and make their already poor physical condition worse. Therefore, they also urgently need drugs that can relieve symptoms and reduce the risk of recurrence.
ZINPLAVA is such a drug.As a type of monoclonal antibody, it can bind to toxin B of Clostridium difficile to neutralize its effects. Therefore, researchers hope that it can alleviate the symptoms of Clostridium difficile infection.
