适 应 症
单一铁螯合物治疗慢性铁超负荷，急性铁中毒，透析病人铝超负荷，诊断铁或铝超负荷。
用法用量
根据个体情况而定，取决于适应症和病情严重程度。剂量范围：慢性铁超20－60mg/kg体重/日。急性铁中毒不超过80mg/kg体重/日。慢性铝超负荷5mg/kg体重,每周一次。铁超负荷试验500mg，铝超负荷静滴试验5mg/kg体重。
不良反应
常见注射部位症状，常伴有关节痛、肌痛、头痛、荨麻诊和发热；发育迟缓和髀改变不常见；少见视觉、沂力障碍；罕见急性呼吸窘迫综合征；头晕、透析性脑病；周围性感觉、运动或混合型神经病变；肾功能损害；血象异常；广泛性皮疹、过敏性/过敏样反应，对耶尔森氏菌和毛霉菌病易感。
禁 忌
已知对活性物质过敏者，不包括后进行治疗的患者。
注意事项
快速静脉滴注：本药溶液的浓度大于10%可引起肾功能损害；视力与听力障碍；发育迟缓，特别是3岁以下儿童刚开始治疗时；急性呼吸窘迫综合征，特别是血清铁浓度偏低，并大剂量使用本药后。治疗前和治疗中应进行眼科和听力的测试，对儿童要监测体重和身高。铁超负荷使人体对耶尔森氏菌小肠结肠炎和耶尔森氏菌假结核类感染的易感性增加。假如应用本药过程中发生此类感染，应立即停止用药，直至感染成功治愈。已有极少数毛霉病病例报告，此时应停用本药并立即给予抗真菌治疗。大剂量的本药使铝相关脑病病人的神经功能障碍恶化。
孕妇及哺乳期妇女用药
妊娠期、哺乳期妇女慎用。
药物相互作用
甲哌氯丙嗪、维生素C、放射性镓67.
药代动力学
去铁胺（DFO）经肌肉或皮下注射后吸收迅速，但由于完整肠的存在，胃肠道的吸收不好。如加入透析液中，则可在腹膜透析间吸收。按10mg/kg体重给健康志愿者肌肉注射DFO后30分钟，血浆浓度达高峰，为5.5umol/L(8.7ug/ml).注射后1小时，铁胺（FO）高峰浓度为3.7umol/L(2.3ug/ml)。体外试验DFO与血清蛋白质的结合少于10%。4种DFO的代谢产物已从铁负荷重的患者尿中分离并检测出来。对健康志愿者的研究表明，DFO和FO均呈双相清除。在第一期（快速），DFO的半衰期为1小时，FO为2.4小时；在第二期（缓慢），两者的半衰期均为6小时。
药理毒理
甲磺酸去铁胺是一种螯合剂，主要与三价铁离子和铝离子形成复合物。对二价离子如欣Fe2+、铜Cu2+、锌Zn2+、钙Ca2+的亲和力很低。由于其螯合特性，无论是游离的，或者与欣蛋白和含铁血黄素结合的铁离子，甲磺酸去铁胺均能与之结合，形成欣胺复合物。由于铁胺与铝胺这两种复合物可完全排出，四磺酸去铁胺能促进铁和铝从小便和粪便中排泄，并因此而减少铁和铝在器官的病理性沉积。然而，甲磺酸去铁胺不能从转铁蛋白、血红蛋白或其它含有血黄素的物质中去除铁离子。

Desferal is used for:
Treating acute iron poisoning and chronic iron overload caused by anemias that require frequent blood transfusions.
Desferal is an iron-chelating agent. It works by binding to excess iron in the blood so that it can be removed by the kidneys and bowels.
Do NOT use Desferal if:
you are allergic to any ingredient in Desferal
you have severe kidney problems or you are unable to urinate
Contact your doctor or health care provider right away if any of these apply to you.
Before using Desferal :
Some medical conditions may interact with Desferal . Tell your doctor or pharmacist if you have any medical conditions, especially if any of the following apply to you:
if you are pregnant, planning to become pregnant, or are breast-feeding
if you are taking any prescription or nonprescription medicine, herbal preparation, or dietary supplement
if you have allergies to medicines, foods, or other substances
if you have brain or kidney disease or heart failure
if you take vitamin C
Some MEDICINES MAY INTERACT with Desferal . Tell your health care provider if you are taking any other medicines, especially any of the following:
Prochlorperazine because loss of consciousness may occur
Gallium-67 because its effectiveness may be decreased by Desferal

How to use Desferal :
Use Desferal as directed by your doctor. Check the label on the medicine for exact dosing instructions.
Desferal is usually administered as an injection at your doctor"s office, hospital, or clinic. If you are using Desferal at home, carefully follow the injection procedures taught to you by your health care provider.
If Desferal contains particles or is discolored, or if the vial is cracked or damaged in any way, do not use it.
If you miss a dose of Desferal , use it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not use 2 doses at once.
Ask your health care provider any questions you may have about how to use Desferal .
Important safety information:
Desferal may cause dizziness or changes in vision. Do not drive, operate machinery, or do anything else that could be dangerous until you know how you react to Desferal . Using Desferal alone, with certain other medicines, or with alcohol may lessen your ability to drive or perform other potentially dangerous tasks.
Do not take large doses of vitamin C without first checking with your doctor.
Desferal may turn your urine a reddish color. This is harmless and not a cause for concern.
Lab tests, including eye exams and hearing tests, may be performed to monitor your progress or to check for side effects. Be sure to keep all doctor and lab appointments.
The weight and growth of CHILDREN receiving Desferal should be monitored every 3 months.
Use Desferal with extreme caution in CHILDREN younger than 3 years of age. Safety and effectiveness in this age group have not been confirmed.
PREGNANCY and BREAST-FEEDING: If you become pregnant, discuss with your doctor the benefits and risks of using Desferal during pregnancy. It is unknown if Desferal is excreted in breast milk. Do not breast-feed while taking Desferal .
Possible side effects of Desferal :
All medicines may cause side effects, but many people have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:
Diarrhea; dizziness; headache; leg cramps; nausea; stomach upset; vomiting.
Seek medical attention right away if any of these SEVERE side effects occur:
Severe allergic reactions (rash; hives; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); change in the amount of urine produced; decreased hearing; fainting; fast heartbeat; fever, chills, or sore throat; muscle or joint pain; numbness or tingling of the face, hands, feet, or skin; pain, burning, swelling, blistering, or redness at the injection site; ringing in the ears; seizures; severe dizziness; unusual bruising or bleeding; vision changes.

Desferal is usually handled and stored by a health care provider. If you are using Desferal at home, store Desferal as directed by your pharmacist or health care provider. Keep Desferal out of the reach of children and away from pets.
