Treatment of advanced renal cell carcinoma.

Dosage |
25 mg IV infusion over 30-60 min once a wk. Administer prophylactic diphenhydramine 25-50 mg IV (or similar antihistamine) approx 30 min before the start of each dose.

Special Precautions |
Hypersensitivity to temsirolimus or metabolites (including sirolimus) or polysorbate 80. Monitor glucose & lipid profiles, renal function at baseline & during therapy, for symptoms or radiographic changes of interstitial lung disease. May result in immunosuppression & bowel perforation. Peri-op period. Increased risk of intracerebral bleeding in patient w/ CNS tumors (primary CNS tumor or metastases) &/or receiving anticoagulants. Avoid live vaccinations & close contact w/ those who have received live vaccines. Pregnancy & lactation.

Adverse Drug Reactions |
Rash, asthenia, mucositis, nausea, edema & anorexia. Anemia, hyperglycemia, hyperlipemia, hypertriglyceridemia, lymphopenia, elevated alkaline phosphatase or serum creatinine, hypophosphatemia, thrombocytopenia, elevated AST & leukopenia.

Drug Interactions |
Strong CYP3A4/5 inducers (dexamethasone, carbamazepine, phenytoin, phenobarb, rifampin, rifabutin & rifampacin). St. John's wort. Strong CYP3A4 inhibitors (atazanavir, clarithromycin, indinavir, itraconazole, ketoconazole, nefazodone, nelfinavir, ritonavir, saquinavir & telithromycin). Grapefruit juice. Sunitinib. Live vaccines.
