Indications Endocrine, rheumatic & haematologic disorders. Collagen dermatologic, ophth, GI, resp & neoplastic disease. Allergic & oedematous states. Acute exacerbation of multiple sclerosis. TB meningitis w/ subarachnoid block. Trichinosis w/ neurologic or myocardial involvement. Shock, acute allergic disorders.

Dosage Life-threatening conditions 30 mg/kg IV over at least 30 min, repeat every 4-6 hr for up to 48 hr. Pulse therapy in RA 1 g/day for 1-4 days or 1 g mthly for 6 mth. Prevention of nausea & vomiting in chemotherapy 250 mg 1 hr before &, pre- & post-chemotherapy. Acute spinal cord injury IV bolus 30 mg/kg over 15 min then pause for 45 min followed by continuous infusion of 5.4 mg/kg/hr for 23 hr. ECG monitoring & defibrillator machine stand-by needed. Other conditions 10-500 mg, doses >250 mg should be given over at least 30 min, subsequent doses, IV or IM, titrated according to conditions & response. Childn & infant Min dose: 0.5 mg/kg every 24 hr. Subsequent doses, IV or IM, titrated according to conditions & response.

Special Precautions Ocular herpes simplex; psychic derangements; nonspecific ulcerative colitis; active or latent peptic ulcer, renal insufficiency, HTN, osteoporosis, myasthenia gravis; immunisation (defer); active or latent TB; unusual stress; pregnancy, lactation. May mask signs of infection & new infection may appear. Posterior subcapsular cataracts & glaucoma w/ possible damage to optic nerves may occur. Reduce dose gradually.

Adverse Drug Reactions GI upsets, aseptic necrosis of femoral & humeral heads; peptic ulceration w/ possible perforation & hemorrhage; manifestations of latent DM; anaphylactic reactions w/ or w/o circulatory collapse, cardiac arrest or arrhythmias, bronchospasm. Fluid & electrolyte disturbances.
