Caspofungin acetate |
Indications |
Empirical therapy for presumed fungal infections in febrile, neutropenic patients. Treatment of invasive candidiasis, including candidemia, esophageal candidiasis & invasive aspergillosis in patients refractory to or intolerant of other therapies. |
Dosage |
Empirical therapy 70 mg as a single loading dose on day 1, thereafter 50 mg daily. Treat for a min of 14 days if positive for fungal infection. Continue treatment for at least 7 days after both neutropenia & clinical symptoms are resolved. Invasive candidiasis 70 mg as a single loading dose on day 1, thereafter 50 mg daily. Continue therapy for at least 14 days after the last positive culture. Esophageal candidiasis 50 mg daily. Invasive aspergillosis 70 mg as a single loading dose on day 1, thereafter 50 mg daily. Should be administered by slow IV infusion over 1 hr. Moderate hepatic insufficiency 35 mg daily followed by a 70-mg loading dose on day 1. |

Avoid concomitant use w/ cyclosporin. Moderate to severe hepatic impairment. Pregnancy & lactation. Childn <18 yr. |
Adverse Drug Reactions |
Fever, headache, abdominal pain, pain, chills; nausea, diarrhea, vomiting; elevated liver enzyme levels; increased serum creatinine; anemia; tachycardia; phlebitis/thrombophlebitis, infused-vein complication, flushing; dyspnea; rash, pruritus, sweating. Anaphylaxis.
