Clarithromycin |
Indications |
Klacid Upper & lower resp tract infections, paed acute otitis media, skin & soft tissue infections, disseminated or localized mycobacterial infections. Eradication of H. pylori in the presence of acid suppression, resulting in decreased recurrence of duodenal ulcer. Treatment of odontogenic infections. Klacid MR Upper & lower resp tract infections, skin & soft tissue infections.

Dosage |
Klacid filmtab Adult 1 tab bd, increased to 500 mg bd in more severe infection. Usual duration: 5-14 days. Renal impairment CrCl <30 mL/min ½ the adult dose. Mycobacterial infections 500 mg bd. MAC prophylaxis 500 mg bd. Odontogenic infection 1 tab bd for 5 days. H. pylori eradication Dual therapy: 500 mg tds w/ omeprazole 40 mg daily for 14 days, followed by omeprazole 20 or 40 mg daily for an additional 14 days; or 500 mg tds w/ lansoprazole 60 mg daily for 14 days. Triple therapy: 500 mg bd w/ lansoprazole 30 mg bd & amoxicillin 1,000 mg bd for 10 days; or 500 mg bd w/ amoxicillin 1 g & omeprazole 20 mg bd for 7-10 days. Klacid paed susp 7.5 mg/kg bd, up to 500 mg bd in nonmycobacterial infections. Usual duration: 5-10 days. Renal impairment CrCl <30 mL/min Reduce by ½. Mycobacterial infections 15-30 mg/kg in 2 divided doses. Klacid IV 1 g daily in 2 divided doses, each infused as bolus or IM inj over 60 min. Klacid MR Adult 500 mg once daily. In more severe infections, increase dose to 1,000 mg once daily. Usual duration: 6-14 days. Moderate renal function (CrCl 30-60 mL/min) Max: 1 tab/day.

May be taken with or without food (MR tab: Take w/ meals. Swallow whole, do not chew/crush.). |
Contraindications |
Known hypersensitivity to macrolides. Concomitant administration w/ astemizole, cisapride, pimozide, terfenadine & ergotamine or dihydroergotamine.

Special Precautions |
Impaired hepatic function & moderate to severe renal failure. Pseudomembranous colitis. Possibility of cross-resistance between clarithromycocin & macrolides as well as lincomycin & clindamycin.

Adverse Drug Reactions |
Diarrhea, vomiting, abdominal pain, dyspepsia, nausea, headache, taste perversion, transient elevation of liver enzymes. Inj site inflammation, tenderness, phlebitis & pain.

Pregnancy Category (US FDA) |
