Indications |
Adult only Lower resp tract, intra-abdominal, biliary tract & gynaecological infections, septicaemia. Surgical prophylaxis in intra-abdominal surgery. Paed only Pneumonia & bacterial meningitis. Adult & paed Pneumonia, complicated & uncomplicated urinary tract, biliary tract & skin & skin structure infections, empiric treatment in febrile neutropenia.

Dosage |
Adult & childn > 40 kg Mild to moderate UTI 0.5-1 g IM/IV 12 hrly. Mild to moderate infections other than UTI 1g IM/IV 12 hrly. Severe infections 2 g IV 12 hrly. Very severe or life-threatening infections 2 g IV 8 hrly. Surgical prophylaxis 2 g as a single IV infusion dose starting 60 min pre-op. Administer a single metronidazole 500 mg IV dose immediately following completion of the Maxipime infusion. If surgical procedure lasts >12 hr from initial prophylactic dose, administer a 2nd dose of Maxipime followed by metronidazole 12 hr after the initial prophylactic dose. Childn >2 mth & ≤40 kg Pneumonia, UTI, skin & skin structure infection 50 mg/kg 12 hrly for 10 days. More severe infections 50 mg/kg 8 hrly for 10 days. Bacterial meningitis & empiric treatment of febrile neutropenia 50 mg/kg 8 hrly for 7-10 days. Childn >40 kg Adult dose. Max: 2 g 8 hrly.

Hypersensitivity to cephalosporins, penicillins or other β-lactam antibiotics.

Special Precautions |
Hypersensitivity, pseudomembranous colitis & superinfection may occur. Renal impairment. Patient who develops diarrhea. Discontinue therapy if allergic reaction occurs. Concurrent treatment w/ nephrotoxic drugs eg aminoglycosides & potent diuretics. Pregnancy & lactation. Elderly.

Adverse Drug Reactions |
Rash, pruritus, urticaria; nausea, vomiting, oral moniliasis, diarrhea; pseudomembranous colitis; headache; fever, vaginitis, erythema, inj site reactions. Elevations in ALT & AST, alkaline phosphatase, total bilirubin, anemia, eosinophilia, prolonged prothrombin time, partial thromboplastin time. +ve Coombs' test w/o hemolysis. Transient elevations of BUN &/or serum creatinine & transient thrombocytopenia, leukopenia & neutropenia.

Pregnancy Category (US FDA) |
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
