Dosage |
Adult ≥60 kg w/ CrCl ≥60 mL/min 400 mg once daily, 30-59 mL/min 200 mg once daily, <29 mL/min 125 mg once daily. Adult <60 kg w/ CrCl 60 mL/min 250 mg once daily, <59 mL/min 125 mg once daily. Patient requiring continuous ambulatory peritoneal dialysis (CAPD) or hemodialysis 125 mg once daily. Concomitant therapy w/ tenofovir disoproxil fumarate Adult ≥60 kg w/ CrCl ≥60 mL/min 250 mg once daily, <60 kg w/ CrCl ≥60 mL/min 200 mg once daily.

Administration |
Should be taken on an empty stomach (Take on an empty stomach ½ hr before or 2 hr after meals. Swallow whole, do not chew/crush.). |

Special Precautions |
Pancreatitis (discontinue). Patient w/ advanced HIV disease, history of neuropathy, treated w/ neurotoxic drugs including stavudine. Fat re-distribution, hyperuricemia. Hepatic & renal impairment. Pregnancy. Elderly. Childn.

Adverse Drug Reactions |
Diarrhea, peripheral neuropathy, nausea, headache, rash, vomiting, pancreatitis. Abdominal pain, alopecia, anaphylactoid reaction, asthenia, chills/fever, pain, fat re-distribution; anorexia, dyspepsia, flatulence; sialoadenitis, parotid gland enlargement, dry mouth & eyes; anemia, leukopenia, thrombocytopenia; symptomatic hyperlactatemia/ lactic acidosis & hepatic steatosis, hepatitis & liver failure; DM, elevated alkaline phosphatase, amylase, γ-glutamyltransferase & uric acid serum levels, hypoglycemia & hyperglycemia; myalgia, rhabdomyolysis including acute renal failure & hemodialysis, arthralgia & myopathy; retinal depigmentation & optic neuritis.

Drug Interactions |
Co-administration w/ drugs that are known to cause peripheral neuropathy or pancreatitis may increase the risk of these toxicities. Allopurinol, ganciclovir, methadone, tenofovir disoproxil fumarate; neurotoxic drugs, ribavirin, alcohol.

Pregnancy Category (US FDA) |
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
