Indications |
Parenteral nutrition for patients & childn >2 yr when oral or enteral nutrition is impossible, insufficient or contraindicated.

Dosage |
Individualised dosage. Adult & childn >10 yr Nitrogen requirements in the normal nutritional state or in mild metabolic stress 0.1-0.15 g nitrogen/kg/day. Nitrogen requirements in moderate to high metabolic stress w/ or w/o malnutrition 0.15-0.3 g nitrogen/kg/day. Childn 2-10 yr Kabiven Initially 12.5-25 mL/kg, increased by 10-15 mL/kg/day up to max dose of 40 mL/kg/day. Infusion rate should not exceed 2.6 mL/kg/hr. Kabiven Peripheral Initially 14-28 mL/kg, increased by 10-15 mL/kg/day up to max dose of 40 mL/kg/day. Infusion rate should not exceed 3.7 mL/kg/hr.

Overdosage |

Contraindications |
Hypersensitivity to egg- or soy protein. Severe hyperlipaemia, severe hepatic insufficiency, severe coagulopathy, inborn errors of amino acid metabolism, severe renal insufficiency w/o access to haemofiltration or dialysis, acute shock, hyperglycemia requiring insulin 6 units/hr, pathologically elevated serum electrolyte levels, acute pulmonary oedema, hyperhydration, decompensated cardiac insufficiency & hypotonic dehydration, hemophagocytotic syndrome. Unstable conditions eg severe posttraumatic conditions, uncompensated diabetes, acute MI, metabolic acidosis, severe sepsis, hyperosmolar coma. Childn & infant <2 yr.

Special Precautions |
Monitor ability to eliminate fat. Correct disturbances of electrolyte & fluid balance before starting infusion. Impaired lipid metabolism eg in renal insufficiency, uncompensated DM, pancreatitis, hepatic impairment, hypothyroidism (w/ hypertriglyceridaemia), sepsis. Regular monitoring of serum glucose, electrolytes, osmolarity, fluid balance, acid-base status & liver enzyme tests. Monitor blood cell count & coagulation when fat is given for a longer period. Phosphate & K intake should be controlled in patients w/ renal insufficiency. Metabolic & lactic acidosis, insufficient cellular O2 supply, increased serum osmolarity, predisposition to electrolyte retention, malnourished patients & those w/ hyperglycaemia. Do not give w/ blood products.

Adverse Drug Reactions |
Hyperthermia, shivering, chills, nausea, vomiting, transient increase in liver enzymes, thrombophlebitis.
