Desmopressin acetate |
Indications |
Shortening or normalization of prolonged bleeding time prior to an invasive operation; prolonged bleeding time as a consequence of congenital or drug-induced thrombocyte dysfunction, uraemia, cirrhosis of the liver or unknown aetiology. Therapeutic control of bleeding & bleeding prophylaxis in patients w/ haemophilia A & von Willebrand's disease.

Dosage |
0.3 mcg/kg SC or IV infusion over 15-30 min. If a positive effect is obtained, the initial dose may be repeated 1-2 times w/ intervals of 6-12 hr.

Contraindications |
Habitual or psychogenic polydipsia; unstable angina pectoris; decompensated cardiac insufficiency; von Willebrand's disease type IIB.

Special Precautions |
Avoid fluid overload & hyponatremia in the very young & elderly patient, conditions characterized by fluid &/or electrolyte imbalance, patient at risk of increased intracranial pressure & those requiring diuretic treatment.

Adverse Drug Reactions |
Fatigue, headache, nausea & stomach pain. At the time of administration, transient fall in BP w/ reflex tachycardia & facial flushing. Rarely, dizziness.

Drug Interactions |
Indomethacin may augment the magnitude but not the duration of the response to desmopressin.

Pregnancy Category (US FDA) |
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
