Indications |
Treatment of symptoms of depressive illness, treatment of obsessive-compulsive disorder (OCD), panic disorder w/ or w/o agoraphobia, posttraumatic stress disorder (PTSD), premenstrual dysphoric disorder (PMDD), social anxiety disorder.

Dosage |
Adult Depression & OCD Initially 50 mg/day. Panic disorder, PTSD, social anxiety disorder Initially 25 mg/day. Increase to 50 mg daily after 1 wk. All dose change be made at intervals of at least 1 wk, up to max 200 mg/day. PMDD Initially 50 mg/day throughout the menstrual cycle or limited to the luteal phase. Dose increased up to 150 mg/day (at 50-mg increments/menstrual cycle) or 100 mg/day during luteal phase. OCD Childn 13-17 yr Initially 50 mg/day, 6-12 yr Initially 25 mg/day, increased to 50 mg/day after 1 wk. Subsequent dose titration may be made in 50-mg/day increments up to a max of 200 mg/day.

Special Precautions |
Monitor for signs & symptoms of serotonin syndrome or neuroleptic malignant syndrome. Co-administration w/ other serotonergic drugs. Switching from SSRIs, antidepressants or antiobsessional drugs. Activation of mania/hypomania. Seizures. Suicide. Abnormal bleeding/haemorrhage. Hyponatremia. Hepatic & renal impairment. May impair ability to drive or operate machinery. Pregnancy & lactation.

Adverse Drug Reactions |
Diarrhea/loose stools, dry mouth, dyspepsia, nausea, anorexia, dizziness, somnolence, tremor, insomnia, sexual dysfunction, increased sweating.

MAOIs, CNS depressants, alcohol, lithium, phenytoin, pimozide, sumatriptan, warfarin, diazepam, tolbutamide.

Pregnancy Category (US FDA) |
Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
