Dosage |
Major depressive episodes 10 mg once daily. Panic disorder w/ or w/o agoraphobia Initially 5 mg on the 1st wk. Then increase dose to 10 mg daily. Social anxiety disorder 10 mg once daily. Dose may be decreased to 5 mg. Generalised anxiety disorder 10 mg once daily. Obsessive compulsive disorder Initially 10 mg once daily. Max (for all indications): 20 mg daily. Elderly >65 yr Initially ½ the usual recommended dose. Consider lower max dose. Reduced renal function Adjust dose. Reduced hepatic function/poor CYP2C19 metabolisers Initially 5 mg daily for the first 2 wk. Max: 10 mg daily. |

Concomitant treatment w/ nonselective, irreversible MAOI.Childn <18 yr. |
Special Precautions |
Paradoxical anxiety, unstable epilepsy, history of mania/hypomania, diabetes, suicide/suicidal ideation, patients at risk of developing hyponatraemia, concomitant use of oral anticoagulants, drugs affecting platelet function, patients w/ bleeding tendencies, concurrent administration w/ electroconvulsive therapy, St. John's wort, CHD. Discontinue if patient develops serotonin syndrome. Avoid abrupt w/drawal esp during pregnancy. Lactation. |
Adverse Drug Reactions |
Decreased appetite or libido, anorgasmia (women), insomnia, somnolence, dizziness, taste disturbance, sleep disorder, sinusitis, yawning, nausea, diarrhoea, constipation, increased sweating, ejaculation disorder, impotence, fatigue, pyrexia.

Drug Interactions |
MAOI, selegiline, serotonergic drugs (eg tramadol, sumatriptan & other triptans), drugs which lower the seizure threshold, lithium, tryptophan, St. John's wort, oral anticoagulants, alcohol. CYP2C19 inhibitors (eg omeprazole, esomeprazole, fluoxetine, fluvoxamine, lansoprazole, ticlopidine), cimetidine, CYP2D6 inhibitors eg flecainide, propafenone & metoprolol, other CNS-acting drugs.
