Dosage |
Essential HTN Initially 10 mg once daily. Maintenance: 20 mg once daily. Max: 80 mg/day. Renovascular HTN Initially 2.5-5 mg once daily. CHF Initially 2.5 mg once daily. Maintenance: 5-20 mg once daily. Acute MI 5 mg w/in 24 hr of the onset of symptoms, 5 mg after 24 hr, 10 mg after 48 hr & then 10 mg once daily thereafter for 6 wk. Renal complications of DM 10-20 mg daily.

Angioedema associated w/ ACE inhibitors. Hereditary or idiopathic angioedema. Pregnancy.

Special Precautions |
Symptomatic hypotension particularly in vol depleted patients & CHF. Hypotension may occur during surgery/anaesth or acute MI. Patients w/ ischaemic heart or cerebrovascular disease, aortic stenosis, hypertrophic cardiomyopathy. Renal impairment. History of angioedema. Lactation. Anaphylactoid reactions reported in patients undergoing hemodialysis & desensitization treatments. Diuretic-treated patients. Higher rate of angioedema in Black patients.

Adverse Drug Reactions |
Dizziness, headache, diarrhoea, fatigue, cough, bronchospasm, nausea, rash, orthostatic effects, asthenia. Rarely, angioneurotic oedema of face, extremities, lips, tongue, glottis & larynx. Very rarely, interstitial angioedema, MI or CVA, possibly secondary or excessive hypotension in high risk patients, palpitation, tachycardia, indigestion, vomiting, paraesthesia, vertigo, pancreatitis, taste disturbance, sleep disturbance, rhinitis, sinusitis, alopecia, psoriasis, severe skin disorders, abdominal pain, dry mouth, hepatitis, jaundice, mood alterations, mental confusion, urticaria, pruritus, diaphoresis, uraemia, oliguria/anuria, renal dysfunction, acute renal failure, impotence, a symptom complex which may include fever, vasculitis, myalgia, arthralgia/arthritis, positive ANA, elevated ESR, eosinophilia & leukocytosis, rash, photosensitivity, alopecia or other dermatological manifestations. Increases (usually reversible) in blood urea, serum creatinine, liver enzymes & serum bilirubin. Small decreases in haemoglobin & haematocrit. Hyperkalaemia, hyponatraemia, leucopenia, thrombocytopenia, anaemia. Rarely, agranulocytosis, haemolytic anaemia.

Drug Interactions |
Risk of hyperkalaemia w/ K-sparing diuretics, K supplements, K-containing salt substitutes. Diuretic induced hypokalaemia may be ameliorated when given w/ a K-losing diuretic. Antihypertensive effect may be diminished by indomethacin. Patients w/ compromised renal function on NSAIDs may result in further deterioration in renal function. Additive antihypertensive effects w/ diuretics. May reduce lithium elimination. May potentiate hypoglycaemic effect of insulin & oral antidiabetic drugs.

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
Category D: There is positive evidence of human foetal risk, but the benefits from use in pregnant women may be acceptable despite the risk (e.g., if the drug is needed in a life-threatening situation or for a serious disease for which safer drugs cannot be used or are ineffective). |
