Dosage |
Adult ≥18 yr 20 mg tds. Renal (CrCl <30 mL/min)/hepatic impairment (Child-Pugh A & B) Downward dose adjustment to 20 mg bd.

Contraindications |
Co-administration w/ nitric oxide donors or nitrates in any form. Combination w/ ketoconazole, itraconazole, ritonavir. Severe hepatic impairment, recent history of stroke or MI, severe hypotension (BP <90/50 mmHg) at initiation. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption.

Special Precautions |
Severe pulmonary arterial HTN (class IV); patient at risk of non-arteritic anterior ischemic optic neuropathy (NAION); known hereditary degenerative retinal disorders eg retinitis pigmentosa; hypotension; fluid depletion; severe left ventricular outflow obstruction or autonomic dysfunction; anatomical deformation of the penis; conditions predisposing to priapism; bleeding disorders or active peptic ulceration. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn & adolescents <18 yr.

Adverse Drug Reactions |
Headache, flushing, diarrhoea, dyspepsia, limb pain.

Drug Interactions |
β-blockers, ketoconazole, itraconazole, ritonavir, saquinavir, erythromycin, carbamazepine, phenytoin, phenobarb, St. John's wort, rifampicin, cimetidine, grapefruit juice, nicorandil, bosentan, amlodipine, α-blockers. Potentiates hypotensive effect of nitrates.

Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters). |
