Gemcitabine HCl

Indications

Locally advanced or metastatic bladder cancer in combination w/ cisplatin, locally advanced or metastatic pancreatic adenocarcinoma. Locally advanced or metastatic non-small cell lung cancer (NSCLC). Locally advanced or metastatic epithelial ovarian carcinoma who have relapsed following recurrence-free interval of at least 6 mth after platinum-based 1st line therapy, in combination w/ carboplatin. Unresectable table, locally recurrent metastatic breast cancer who have relapsed following adjuvant/neoadjuvant chemotherapy in combination w/ paclitaxel.

Dosage

NSCLC 1,000 mg/m² by 30-min IV infusion once wkly for 3 wk followed by 1 wk rest. The 4-wk cycle is then repeated. Combination w/ cisplatin: 1,250 mg/m² as 30-min IV infusion on day 1 & 8 of each 21-day cycle. Pancreatic adenocarcinoma 1,000 mg/m² by 30-min IV infusion once wkly for up to 7 wk followed by 1 wk rest. Subsequent cycles: Once wkly for 3 consecutive wk out of every 4 wk. Bladder cancer 1,000 mg/m² by 30-min IV infusion on days 1, 8 & 15, of each 28-day cycle, in combination w/ cisplatin 70 mg/m² on day 1. Breast cancer Paclitaxel 175 mg/m² as 3 hr IV infusion on day 1, followed by gemcitabine 1,250 mg/m² by 30-min IV infusion on days 1 & 8 of each 21-day cycle.Ovarian cancer 1,000 mg/m² by 30-min IV infusion on days 1 (in combination w/ carboplatin) & 8 of each 21-day cycle.

Overdosage

Known hypersensitivity to gemcitabine. Lactation.

Special Precautions

Monitor platelet, leukocyte & granulocyte counts. Impaired bone marrow function, renal or hepatic impairment. Concomitant use w/ radiotherapy. Live vaccines, CV events, pulmonary effects (discontinue therapy), controlled Na diet. May impair ability to drive or operate machinery. Male infertility. Pregnancy. Childn.

Adverse Drug Reactions

Leucopenia, thrombocytopaenia, anaemia, dyspnoea, nausea, vomiting, elevated liver transaminases & alkaline phosphatase, allergic skin rash, alopecia, haematuria, mild proteinuria, flu-like symptoms, oedema/peripheral oedema.
