Dosage |
Complicated skin & skin structure infections, community-acquired pneumonia including concurrent bacteremia, nosocomial pneumonia Adult & adolescent ≥12 yr 600 mg IV or PO 12 hrly. Childn from birth-11 yr 10 mg/kg IV or PO 8 hrly. All doses to be given for 10-14 days. Vancomycin-resistant Enterococcus faecium infections including concurrent bacteremia Adult & adolescent ≥12 yr 600 mg IV or PO 12 hrly. Childn from birth-11 yr 10 mg/kg IV or PO 8 hrly. All doses to be given for 14-28 days. Uncomplicated skin & skin structure infections Adult 400 mg PO 12 hrly. Adolescent 600 mg PO 12 hrly. Childn 5-11 yr 10 mg/kg PO 12 hrly; <5 yr 10 mg/kg PO 8 hrly. All doses to be given for 10-14 days.

Contraindications |
Hypersensitivity. Concomitant MAOIs (eg phenelzine, isocarboxacid), directly & indirectly acting sympathomimetics (eg pseudoephedrine), vasopressive agents (eg epinephrine, norepinephrine), dopaminergic agents (eg dopamine, dobutamine), serotonin re-uptake inhibitors, TCAs, serotonin 5-HT1 receptor agonists (triptans), meperidine & buspirone. Patients w/ uncontrolled HTN, pheochromocytoma, thyrotoxicosis; carcinoid syndrome; catheter-related blood stream or catheter site infections.

Special Precautions |
Monitor CBC wkly in patients who receive linezolid for >2 wk w/ preexisting myelosuppression, receive concomitant drugs that produce bone marrow suppression or w/ chronic infection who receive previous or concomitant antibiotics. Discontinue if myelosuppression develops or worsens. Clostridium difficile-associated diarrhea must be considered in patient w/ diarrhea following antibiotic use. Lactic acidosis. Monitor for signs & symptoms of serotonin syndrome eg cognitive dysfunction, hyperpyrexia, hyperreflexia & incoordination. Peripheral & optic neuropathy. Monitor visual function. Convulsion. Overgrowth of nonsusceptible microorganisms. Uncontrolled HTN, pheochromocytoma, carcinoid syndrome, untreated hyperthyroidism. Phenylketonurics. May impair ability to drive or operate machinery. Pregnancy & lactation.

Adverse Drug Reactions |
Diarrhea, headache, nausea. Oral & vag moniliasis, HTN, dyspepsia, localized abdominal pain, pruritus & tongue discolouration.

Drug Interactions |
Adrenergic & serotonergic agents, food or beverage w/ high tyramine content. Strong CYP450 inducers eg rifampin, carbamazepine, phenytoin, phenobarb.

Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available.
