50 to 100 mg/kg/day IV in equally divided doses every 6 to 8 hours (Max: 6g/day).
Intramuscular dosage
Adults
0.75 IM every 8 hours.
Infans 3 months and older, Children, and Adolescents

For the treatment of bone and joint infections.
Intravenous dosage
Adults
1.5 g IV every 8 hours. For life-threatening infections or infections due to less susceptible organisms, 1.5 g IV every 6 hours may be required.
Infants 3 months or older, Children, and Adolescents
150 mg/kg/day IV in equally divided doses every 8 hours (Max: 6 g/day).

250 mg PO every 12 hours for 10 days. Guidelines do not recommended cefuroxime for routine therapy for Group A Streptococcal pharyngitis therapy to prevent rheumatic fever.
Oral dosage (suspension)
Infants 3 months and older and Children
20 mg/kg/day PO divided into 2 doses for 10 days (Max: 500 mg/day). Guidelines do not recommended cefuroxime for routine therapy for Group A Streptococcal pharyngitis therapy to prevent rheumatic fever.
For the treatment of gonorrhea.
For cervicitis or urethritis due to N. gonorrhoeae or rectal gonorrhea in females or urethritis in males.
Oral dosage (tablets)
NOTE: Oral tablets and suspension are not bioequivalent and are not substitutable on a milligram per milligram basis.
Adults and Adolescents
Due to resistance, the CDC does not recommend cefuroxime for gonorrhea. The manufacturer recommends 1 g PO as a single dose.
Intramuscular dosage

The CDC does not recommend IM cefuroxime. The FDA-approved dose is 1.5 g IM as a single dose at 2 different sites in combination with probenecid.
For disseminated gonococcal infection due to N. gonorrhoeae.
Intravenous dosage
Adults and Adolescents
The CDC does not recommend cefuroxime for gonorrhea. The manufacturer recommends 750 mg IV every 8 hours. In severe or complicated infections, a dosage of 1.5 g IV every 8 hours is recommended.
For the treatment of early Lyme disease (erythema migrans).

For the treatment of acute otitis media.
